Clinical Analysis Of Modified Vaginal Posterior Vaginal Tightening And Autologous Fat-filled Vaginal Tightening For Severe Vaginal Relaxation

ObjectiveThrough the clinical manifestations,diagnosis basis and treatment methods of patients with severe vaginal relaxation,the effects of vaginal tightening of autologous fat transplantation and vaginal tightening of modified posterior vaginal wall on severe vaginal relaxation were investigated.MethodsForty-eight patients with severe vaginal relaxation who were admitted to the Department of Plastic Surgery,Binzhou Medical College Hospital from August 2016 to December 2018 were enrolled and analyzed.Patients were divided into two groups,a control group and an observation group.The control group used vaginal tightening with autologous fat transplantation,and the observation group used vaginal tightening to improve the posterior wall of the vagina.Each treatment has advantages and disadvantages.At the same time,intraoperative blood loss,operational complexity,operative time,vaginal tightening,postoperative healing,postoperative complications and postoperative satisfaction were followed up for 1 year to compare the advantages and disadvantages of the two surgical methods.All cases were evaluated before and 6 months after surgery,and 1 year after surgery using the Female Sexual Function Scale(FSFI)and finger test.The scores included sexual desire,sexual arousal,vaginal wetness in sexual life,orgasm.Sexual life satisfaction,sexual life pain,data results using SPSS24.0 statistical software for data analysis.ResultsAll 48 patients were delivered vaginally.The quality of the sexual life declines and the sense of decline;the number of fingers entering the vaginal?4 fingers.All patients were treated with vaginal tightening.It can be known that the vaginal tightening of the modified posterior wall of the vagina preserves the mucosa of the posterior wall of the vagina.The mucosa on the posterior wall of the vagina is rich in sensory nerves,which has a good effect on penile stimulation.At the same time,the procedure can be repaired due to childbirth and pregnancy.Injury of the pelvic floor muscles,intraoperative U-shaped suture of the levator ani muscle,can greatly effectively improve the vaginal relaxation and strengthen the posterior wall of the vagina.Vaginal tightening of autologous fat transplantation has less trauma and less bleeding,but the operation time is long,and there is a certain survival rate and absorption rate of fat.The patient has the possibility of secondary surgery and increases the cost of the patient.No rupture of the incision,infection,nodules,cysts and other related complications were found in the above patients.All the 48 cases were followed up well.There were no cases of loss of follow-up.The preoperative and postoperative were scored accurately and in detail.The difference of the average scores of 6 months after operation and 1 year after operation was analyzed by SPSS24.0.<0.05,the difference was considered statistically significant.All follow-up cases have a good self-report.ConclusionVaginal tightening of autologous fat transplantation and vaginal tightening of the posterior vaginal wall have a certain clinical effect on the treatment of severe vaginal relaxation.However,vaginal tightening of the posterior wall of the vaginal is better for treating severe vaginal relaxation than autologous fat transplantation.Tightening,and sexual life satisfaction was also higher,P<0.05.
