Objective:To investigate the clinical efficacy and safety of duodenum-preserving pancreatic head resection(DPPHR)compared with pancreaticoduodenectomy(PD)in the treatment of benign or low-grade malignant tumors.Methods:A case-control study on DPPHR and PD in the treatment of benign or low-grade malignant tumors published in China National Knowledge Infrastructure,WANFANG data knowledge Service platform,PubMed,EMBASE,MEDLINE,Cochrane Library,China Biomedical Database and other domestic and foreign databases from January 1990 to November 2019 was searched.Meta-analysis was performed on the literature that meet the standards by Review Manager version 5.3.Results:15 articles were included,including 801 cases,including 333 cases of DPPHR and 468 cases of PD.According to the results,there was no significant difference in age,operational time,postoperative hospital stay,hospital mortality,sex,recurrence rate,postoperative pancreatic fistula rate,postoperative pancreatic hemorrhage rate and delayed gastric emptying between the two groups(P>0.05).DPPHR is superior to traditional PD in terms of estimated intraoperative blood loss,postoperative pancreatic endocrine and exocrine dysfunction.Conclusions:DPPHR has the advantages of shortening operation time and protecting endocrine and exocrine function of pancreas in the treatment of benign or low-grade malignant disease.
