Clinical Efficacy Of Duodenum-preserving Pancreatic Head Resection Versus Pancreatoduodenectomy

Abstract/Summary:
Objective: At present,there are very few follow-up studies on the long-term clinical efficacy and complications of duodenum-preserving pancreatic head resection versus pancreatoduodenectomy.Based on the comparative analysis of the general information of the two surgical procedures,the aim of our study was to assess not only the long-term clinical efficacy and complications but also the influence on the quality of life of the patients.Methods: Eighty-eight patients undergoing DPPHR or PD from Jan 2014 to Jan 2019 were retrospectively analyzed and compared.The preoperative variables,postoperative hospital stay,the incidence of long-term postoperative complications were compared.The quality of life and nutrition was assessed by using the EORTC QLQ-C30 and NRS 2002 respectively.Results: There were significant difference in the operation time was(478.39 ±160.88 vs 600.09 ±140.72,P<0.001),duration of hospital stay(25.42 ±12.42 vs 31.37 ±12.38,p = 0.034),cost of hospitalization(9.34 ±3.19 vs 12.82 ±4.75,P<0.001),pancreatic exocrine insufficiency(6.5% vs 36.8%,p = 0.004),postoperative fatty liver(9.7% vs 31.6%,p = 0.041),weight gain(4.03 ±4.42 vs 1.04 ±2.11,P<0.001),long-term complications(32.3% vs 64.9%,P = 0.003),fatigue score(4.66 ±8.97 vs 9.75 ±12.26,P = 0.029),diarrhea score(2.15 ±11.97 vs 15.20 ±20.96,P<0.001)and the overall health status(94.89 ±7.03 vs 88.45 ±10.77,P=0.001).Conclusion:DPPHR offered advantages with regard to the operation time,postoperative recovery,long-term complications,quality of life,nutritional status and so on.In the treatment of chronic pancreatitis with mass in the head of the pancreas,benign,borderline and even low-grade malignant tumors of pancreatic head,patients can benefit more greatly,DPPHR is more effective than PD.
