BackgroundNonalcoholic fatty liver disease(NAFLD)has become the most common chronic liver disease worldwide.It can not only lead to liver disease related deaths,but also be associated with coronary artery disease(CAD),type 2 diabetes and other diseases.NAFLD is associated with an increased risk of CAD,and abnormal lipid metabolism and insulin resistance are common risk factors for both diseases.Genetic factors play an important role in the occurrence and development of NAFLD and CAD.Myeloid-Epithelial-Reproductive Tyrosine Kinase(MERTK)is a tyrosine kinase receptor located on the plasma membrane.Foreign studies have shown that the MERTK rs4374383 A allele can affect the incidence of NAFLD and CAD by improving lipid metabolism and insulin sensitivity,but the correlation between the MERTK rs4374383 gene polymorphism and the risk of NAFLD and CAD in Chinese population is not clear.ObjectiveTo investigate the correlation of MERTK rs4374383 gene polymorphism in nonalcoholic fatty liver and coronary artery disease susceptibility.Methods(1)In this study,a case-control study was used,and the subjects were all from part of the population in Qingdao.In this study,132 NAFLD patients were included in the NAFLD group,148 CAD patients were included in the CAD group,and 112 patients who met both NAFLD and CAD diagnostic criteria were taken as the combined group.At the examination center,213 healthy individuals were randomly selected as the healthy control group.Collect general clinical data of four groups.(2)The fasting blood of the subjects was collected in anticoagulant tubes,and fasting blood glucose(Glu),triglyceride(TG),total cholesterol(TC),high-density lipoprotein(HDL),low-density lipoprotein(LDL),alanine aminotransferase(ALT),aspartate aminotransferase(AST),alanine aminotransferase(GGT),alkaline phosphatase(ALP),total bilirubin(TBIL)were measured.(3)Blood DNA was extracted and the genotype of MERTK rs4374383 was detected by polymerase chain reaction and flight mass spectrometry.(4)Statistical analysis:Hardy-Weinbeurg genetic balance law was used to test whether the four groups of subjects were representative of the population.Measurement data consistent with the normal distribution between the two groups should be analyzed by independent sample t test,and measurement data with non-normal distribution should be analyzed by Wilcoxon rank sum test.Count data were analyzed and compared using the Pearson ?~2 test,and the unconditional Logistic regression model was used to analyze the risk of genotypes between groups.The data analysis in this paper uses SPSS25.0 software,P<0.05 indicates that the difference is statistically significant.Result(1)There was no significant difference in gender,AST and TBIL between NAFLD group,CAD group,combined group and control group(P>0.05),there was significant difference in age,BMI,Glu,TG,TC,HDL,LDL,ALT,GGT and ALP(P<0.05).(2)There were significant differences in genotype and allele distribution among the subjects(P<0.05).(3)Distribution of genotypes of MERTK rs4374383 locus between CAD group and control group,combined group and control group,NAFLD group and combined group,CAD group and combined group,there was statistical difference(P<0.05).The distribution of A and G alleles was statistically different between NAFLD group and control group,CAD group and control group,CAD group and combined group(P<0.05).(4)Chi-square test was used to analyze the incidence of MERTK rs4374383genotypes and alleles among the groups.The analysis showed that:Compared to G allele,The A allele of MERTK rs4374383 locus is the protective factor of NAFLD(OR=0.680,95%CI:0.470?0.983,P=0.040),CAD(OR=0.440,95%CI:0.313?0.619,P<0.001)and NAFLD combined with CAD(OR=0.589,95%CI:0.399?0.871,P=0.008).(5)Application of unconditional Logistic regression analysis shows:Compared with the carriers of GG genotype,AA+AG genotype is related to the risk reduction of NAFLD(OR=0.390,95%CI:0.155?0.982,P=0.046)and NAFLD combined with CAD(OR=0.358,95%CI:0.140?0.919,P=0.033).(6)Comparing the laboratory indicators of those who carried the A allele with those who did not,the GGT level of the A allele carrier was significantly lower than that of the non-carrier(P<0.05)in all subjects.In the control group,the ALT and GGT levels of carriers were significantly lower than those of non-carriers(P<0.05),and there was no statistically significant difference in other indicators(P>0.05).ConclusionAmong some people in Qingdao,the MERTK rs4374383 locus gene polymorphism is associated with susceptibility to NAFLD and CAD diseases.The MERTK rs4374383 locus AA+AG genotype is a protective factor for NAFLD and NAFLD combined with CAD,and the MERTK rs4374383 locus A allele is a protective factor for NAFLD and NAFLD combined with CAD.
