Objective: To investigate the role of human B-cell-activating factor(BAFF)and interleukin-4(IL-4)in the pathogenesis of Graves ’ disease,and evaluate the significance of BAFF and IL-4 in the diagnosis and efficacy evaluation of Graves’ disease.Methods: A total of fifty-eight patients with Graves’ disease(GD group)who were admitted to the outpatient department of endocrinology in our hospital from December 2018 to June 2019 were selected,among then,33 cases(GD1 group)were newly diagnosed,25 cases(GD2 group)were cured,and 27 cases(N group)were healthy during the same period.Clinical data of the three groups were obtained,including biochemical indexes such as TT3,TT4,FT3,FT4,TSH TGAb,TMAb,and previous history,pregnancy,smoking,medication,and thyroid function;Serum levels of BAFF and IL-4 in each group were determined by elisa,the relationship between the clinical data of each group and the changes in the expression levels of BAFF and IL-4 as well as the correlation with each indicator were analyzed.Results: 1.Serum expression levels of BAFF and IL-4 in GD1 group were up-regulated compared with that in GD2 group and compared with that in N group,and the differenc e was statistically significant(p<0.05);There was no significant difference in serum exp ression levels of BAFF and IL-4 between GD2 group and N group(p>0.05).2.There was a significant positive correlation between serum BAFF level and TRAb tit er in GD1 group(r=0.75 p<0.05),while there was no correlation between serum BAFF l evel and FT4 titer in GD1 group and GD2 group(r=0.26 p>0.05;R =0.12 p>0.05).3.Serum IL-4 level in GD1 group was positively correlated with TRAb and FT4 titer(r=0.64 p <0.05;r=0.39 p <0.05),there was no correlation between IL-4 level and FT4 tite r in GD2 group(r=-0.05 p >0.05).4.Serum BAFF level in GD1 group was positively correlated with IL-4 level(r=0.50 p<0.05),there was no correlation between the two in the serum of GD2 group(r=-0.31 p>0.05).5.Through analyzing receiver operating characteristic cure(ROC),it was found that the AUC of joint diagnosis of Graves’ disease by BAFF and IL-4 was 0.889,indicating th at BAFF and IL-4 have certain diagnostic value for Graves’ disease.Conclusion: 1.The high expression of BAFF and IL-4 in serum of newly diagnosed GD patients suggests that both of them may promote the occurrence of GD.2.The positive correlation between BAFF and IL-4 and the pathogenic antibodies(TRAb)in serum of newly diagnosed GD patients suggests that they may be involved in the pathogenesis of GD.3.The differences in serum expression levels of BAFF and IL-4 between the newly developed GD group and the cured GD group suggest that antithyroid drugs for GD may improve immunity.
