Objective To investigate the relationship between serum complement C1q/tumor necrosis factor-related protein 3(CTRP3)levels and renal damage in diabetic nephropathy.Methods The clinical and laboratory data of patients diagnosed by the Department of Nephrology in the First Affiliated Hospital of Bengbu Medical College.There were41 patients diagnosed with type 2 diabetic nephropathy who met the NKF-KDOQI criteria were selected as group diabetic nephropathy(group DN),then we divided them into 2 subgroups according to estimated glomerular rate filtration(eGFR).(early subgroup,n=21,eGFR30~90ml/min;late subgroup,n=20,eGFR?29ml/min).We selected 30 non-diabetic chronic kidney disease patients as group non-diabetic chronic kidney disease(group Non-DN).And 30 healthy individuals from physical examination center were selected as the normal control group at the same time.(group NC,n=30).We analyzed the relationship between CTRP3 with total cholesterol(TC),triglyceride(TG),high density lipoprotein(HDL),low density lipoprotein(LDL),serum creatinine(SCr),cystatin C(CysC),urea(BUN),high-sensitivity C-reactive protein(hs-CRP)and other indicators,to conclude the difference of CTRP3 levels in different groups and the correlation with the above indicators.Results(1)The serum levels of CTRP3 were lower in the group DN(284.33±32.92 ng/ml)and group Non-DN(314.42±20.82 ng/ml)than the group NC(375.57±18.02),and the group DN was the lowest.The difference was statistically significant.(2)In the DN group,the serum levels of CTRP3 in the eGFR4-5 subjects(274.24±23.6 ng/ml)were significantly lower than eGFR 1-3 subjects(293.95±37.94ng/ml).(3)Significantly,pearson correlation analysis showed that there was a negative correlation between CTRP3 and CysC,BUN and TG(r=-0.266?-0.418?-0.667,P<0.05).There was an obvious positive correlation between CTRP3 and HDL(r=0.452,P<0.05).There was no significant correlation with age,BMI,FBG,UA,TC,LDL(P>0.05).Spearman correlation analysis showed that there was an obvious negative correlation between CTRP3 and SCr and hs-CRP(r_s=-0.460?-0.521,P<0.05).There was a significant positive correlation between CTRP3 and eGFR(r_s=0.508,P<0.05).There was no statistically obvious correlation with gender(P>0.05).(4)Multiple linear regression results showed that TG and eGFR were statistically significant in the model(P<0.05).Conclusion The level of serum CTRP3 in patients with diabetic nephropathy was lower than that in normal people,and the level of CTRP3 gradually decreased with renal dysfunction.The experimental results showed that there was a significant negative correlation between CTRP3 and TG and hs-CRP.TG and eGFR were independent factors of serum CTRP3.Thus,it is speculated that CTRP3 levels are associated with disorders of glucose and lipid metabolism and inflammation;CTRP3 may be involved in the process of renal dysfunction in chronic kidney disease,especially in patients with diabetic nephropathy.
