Objective: Primary Sj?gren's syndrome(pSS)is a chronic autoimmune disease that can involve multiple organs and systems.When the respiratory system is involved,it mainly includes airway abnormalities,interstitial pneumonia(ILD)and diseases related to lymphoid tissue hyperplasia.Interstitial pneumonia is the most common complication involving the respiratory system.This study aims to explore the risk factors of pSS-ILD.Methods: This study retrospectively analyzed 281 inpatients diagnosed with Sjogren's syndrome from May 2014 to May 2018 in the Department of Rheumatology and Immunology of Zhejiang Provincial People's Hospital.There were 27 patients with pSS-ILD group(excluding all patients with pSS who had previous tuberculosis,chronic bronchitis,emphysema,bronchial asthma,bronchiectasis,lung tumors,and other respiratory diseases,and those with chronic heart,brain,liver,and kidney dysfunction).In addition,52 patients with pSS without other system injuries were set as the control group.In this study,the general demographics,clinical manifestations,and laboratory tests of the two groups of patients were analyzed.Logistic regression analysis was used to find the risk factors for the onset of pSS-ILD.At the same time,the imaging characteristics of pSS-ILD patients were analyzed.Results: Through analysis of related factors of patients in pSS group and pSS-ILD group,and imaging analysis of patients in pSS-ILD group.It was found that compared with the patients in the pSS group,the patients in the pSS-ILD group were older and prone to cough and expectoration(P <0.05).C-reactive protein(CRP),erythrocyte sedimentation rate(ESR),lactate dehydrogenase(LDH),rheumatoid factor(RF)are correlated with the incidence of pSS-ILD(P <0.05).All patients in the pSS-ILD group underwent high-resolution CT(HRCT)examination and found that the most commonmanifestations were patchy shadows,followed by grid shadows,which were more common in the lower lobe of both lungs.Through logistic regression analysis,after adjusting for other factors,it is concluded that age and lactate dehydrogenase(LDH)are risk factors for the onset of pSS-ILD.Conclusion: In pSS patients age and elevated lactate dehydrogenase(LDH)are risk factors for the onset of pSS-ILD.The imaging of pSS-ILD is mostly patchy shadows,followed by grid shadows.
