Objective: To investigate the effect of different antipsychotics(olanzapine,quetiapine,risperidone)on thyroid hormone level in female schizophrenic patients,and to observe the incidence of abnormal thyroid hormone after antipsychotics treatment,as well as the difference of three drugs in thyroid hormone.Methods: This study selected female schizophrenia patients who were hospitalized in Shandong Mental Health Center from May 2018 to February 2019 as the research object.According to the grouping criteria and exclusion criteria,179 patients were screened out.They were divided into three treatment groups,group A(treatment with olanzapine system alone)60 cases;59 cases of group B(treatment with quetiapine system alone);60 cases of group C(treatment with risperidone system alone).At the three time points of baseline,weekend 4 and weekend 8,chemiluminescence immunoassay was used to perform five measurements of thyroid function.The indicators included total triiodothyronine(TT3),total thyroxine(TT4),and thyroid stimulating hormone(TSH),free triiodothyronine(FT3),free thyroxine(FT4).All data are analyzed and processed using SPSS22.0 statistical software.Results:1.The indexes of TT3,TT4,FT3,and FT4 of patients in theolanzapine group decreased from baseline at the 4th weekend,and the differences were statistically significant(P<0.05).TSH index increased from baseline,the difference was statistically significant(P<0.05).In the quetiapine group,the TT3,TT4,FT3,and FT4 indexes of the 4th weekend were lower than the baseline,and the difference was statistically significant(P<0.05);the TSH was not statistically different from the baseline(P>0.05).The indexes of TT3,TT4,FT3,and FT4 in the risperidone group decreased from baseline at the 4th weekend,and the differences were statistically significant(P<0.05).TSH index increased from baseline,the difference was statistically significant(P<0.05).2.The TSH in the olanzapine group was higher than that at the 4th weekend,and the difference was statistically significant(P<0.05),there was no significant change in other indicators(P>0.05).The TSH in the quetiapine group was higher than that at the 4th weekend,and the difference was statistically significant(P<0.05),there was no significant change in other indicators(P>0.05).The three indicators of TT3,FT3,and TT4 in the risperidone group at the 8th weekend decreased compared with the 4th weekend,and the difference was statistically significant(P<0.05),there was no statistically significant difference between FT4 and TSH indicators compared with the 4th weekend(P>0.05).3.There is no significant difference among the three drugs on TSH(F=0.26,P=0.77),all of which could increase TSH;the effects of the threedrugs on FT3 and FT4 are significantly different(F=10.03,P=0.00;F=12.96,P=0.00).All the three drugs could cause the decrease of FT3 and FT4,but quetiapine had significantly higher effect on FT3 and FT4 than the other two drugs.4.There was no statistically significant difference in the abnormal rates of TT3,FT3,FT4,and TSH in the three time windows in the olanzapine group(P>0.05).The difference in abnormal incidence rate of TT4 in three time windows was statistically significant(P<0.05),the results showed that the incidence of TT4 abnormality was significantly increased after olanzapine treatment.5.During the course of treatment of quetiapine group,the differences in the abnormal incidence rates of TT3,TT4,FT3,and FT4 in the three time windows were statistically significant(P<0.05),after treatment with quetiapine,the incidence of abnormality of the above indicators increased significantly.There was no significant difference in the abnormal incidence rate of TSH in the three time windows(P>0.05).6.There was no significant difference in the abnormal incidence rates of TT3,TT4,TSH,FT3,and FT4 in the three time window test results of patients in the risperidone group(P>0.05).7.The effect of the three drugs on the incidence of abnormal TSH was not significantly different(P>0.05).The abnormal incidence of FT3 and FT4 caused by quetiapine was significantly higher than that of theother two drugs(P<0.05).Conclusion:1.Quetiapine,risperidone and olanzapine have certain effects on human thyroid function during the treatment of female schizophrenia.The effects of the three drugs on TSH levels were not significantly different,all of which could increase TSH.All the three drugs could cause the decrease of FT3 and FT4,but quetiapine had significantly higher effect on FT3 and FT4 than the other two drugs.2.There is no significant difference in the effect of the three drugs on the abnormal incidence of TSH,and the abnormal incidence of FT3 and FT4 caused by quetiapine is significantly higher than the other two drugs.3.All three antipsychotics have the risk of causing abnormal changes in thyroid function,of which quetiapine has the greatest impact on thyroid function.In the process of using the above drugs,the thyroid hormone levels of patients should be regularly tested.
