| Objective:the purpose of this study is to apply the model of quality control circle to the incidence of choking in elderly inpatients with mental illness and to further evaluate the effect of intervention.Through the activity tool of quality control circle group,analyze the causes of choking in elderly inpatients with mental illness,formulate corresponding improvement measures,and compare the incidence of choking in elderly inpatients with mental illness and psychiatric department before and after the implementation of quality control circle activity To explore the application effect of quality control circle activities in reducing the incidence of choking in the elderly patients with mental illness,so as to reduce the incidence of choking in the elderly patients with mental illness,improve the level of professional technical ability of psychiatric nurses,to avoid adverse events in psychiatric nursing,and to provide effective nursing measures and Countermeasures for psychiatric nurses Examination is conducive to the continuous improvement of nursing quality.Methods:Taking the number of meals for the elderly patients in the psychiatry department of a third level hospital in Anhui Province as the research object,428 elderly inpatients with psychosis who met the standard from February 1 to March 31,2019 as the control group and 428 elderly inpatients with psychosis who met the standard from July 1 to August 31,2019 as the quality control circle Pre group.The patients in the control group were given routine psychiatric nursing,the patients in the quality control circle intervention group were given quality control circle countermeasures,and the activities of the quality control circle group were used to analyze the causes of choking and formulate corresponding countermeasures.Before and after the implementation of the quality control circle intervention measures,the number of cases of choking defects were collected in the psychiatric geriatric ward,and the professional skills of psychiatric nurses were assessed to compare the incidence of choking of elderly inpatients with psychosis before and after the implementation of the quality control circle activities,the professional skills of psychiatric nurses,and evaluate the effect of the quality control circle intervention.Result:1.There was no significant difference between the control group and the quality control circle intervention group in gender,incidence of choking,education level,cognitive status,drug understanding,self-care ability,smoking history,drinking history,combined with basic diseases,combined with drugs other than psychotropic drugs(P>0.05).2.Quality control circle activities significantly reduced the incidence of choking in hospitalized elderly patients with mental illness.In the control group,there were totally 428 times of eating in the psychiatric ward,26 times of choking,including 11 times of choking in schizophrenia,15 times of choking in Alzheimer's disease,and the total choking rate was 6.07%.Among the 428 times of eating in psychiatric ward of quality control circle intervention group,5 times of choking occurred,3 times of choking occurred in schizophrenics,2 times of choking occurred in Alzheimer's disease,and the total choking rate was 1.17%.The results showed that the incidence of choking in inpatients with mental diseases was significantly reduced.The results of control group and quality control circle intervention group were poor The difference was statistically significant(P<0.05).3.Through the statistical analysis of the original data,this paper analyzes the important factors that lead to the occurrence of choking in the elderly inpatients in the psychiatric department,including three factors:eating too fast,swallowing dysfunction and drug influence,and finally analyzes the true causes of choking through these three important factors.4.Through the standard process of quality control circle and the analysis of the true cause by using the methods and tools of quality control circle,the corresponding countermeasures to reduce the incidence of choking in the elderly inpatients in psychiatric department are put forward,which are to strengthen the risk assessment of choking and prolong the time of eating,to improve the effect of patients' eating intervention measures and to strengthen the training of swallowing function treatment,to strengthen the propaganda and education of patients' choking related knowledge,to strengthen the communication of medical care and Training content of drug specialty.5.The activities of quality control circle improve the post competency of psychiatric nurses.On the one hand,the activities of quality control circle help to improve the choking rescue skills of psychiatric nurses.Before and after the implementation of the quality control circle,15 nurses in the same nursing team were assessed for choking rescue operation.The assessment score increased from(81.33±5.41)in the control group to(92.73±1.71)in the quality control circle intervention group.The t-value of the two groups was-7.421(P<0.05),with statistical significance.On the other hand,the activities of quality control circle are helpful to improve the level of specialized theory of psychiatric nursing staff.Before and after the implementation of quality control circle,15 nurses from the same nursing team were assessed on the knowledge of psychotropic drugs.The assessment score was increased from(75.93±4.03)in the control group to(90.87±3.07)in the intervention group.The t-value of the two groups was-10.473(P<0.05)The difference was statistically significant.6.The activity of quality control circle is helpful to improve the comprehensive quality of nursing staff.Through the activities of the quality control circle,the circle members have significantly improved in QCC technique application,team spirit,professional knowledge,communication and coordination ability,activity self-confidence,sense of responsibility and honor,work enthusiasm,and problem-solving ability.The radar chart intuitively shows that the intangible achievements of the quality control circle have achieved remarkable results.Conclusion:1.The application of quality management tools in the quality control circle can reduce the incidence of choking in elderly psychiatric patients,improve the professional skills of psychiatric nurses,and improve the post competency of psychiatric nurses.2.Through the quality control circle activity,it is helpful to improve the comprehensive quality of nursing staff,so that the work enthusiasm,personal expression ability,creative thinking,use of quality control circle techniques and team spirit of quality control circle staff are significantly improved,and the team cohesion of medical staff in the whole department is further improved,which is worth promoting.2.The quality management tools of quality control circle can realize the standardization of clinical nursing workflow,improve the quality of nursing,reduce the incidence of nursing adverse events,reduce the sufferings of patients,help to improve the professional value of nurses and optimize the quality of psychiatric nursing service. |
