Effect Of Ointment Massage On Recovery Of Gastrointestinal Function After Laparoscopic Surgery For Colorectal Cancer

Objective: To explore the effect of ointment massage on the recovery of gastrointestinal function in patients with colorectal cancer after operation,and to evaluate the efficacy,safety and economic benefits of ointment massage,so as to provide clinical basis for promoting the rapid recovery of patients with colorectal cancer after operation.Methods: From January 2019 to December 2019,84 patients who met the inclusion criteria of laparoscopic colorectal cancer surgery in the First Affiliated Hospital Of Guangxi University Of Traditional Chinese Medicine were selected.They were randomly divided into three groups:ointment massage group,massage group and routine group,28 cases in each group.The three groups were intervened on the basis of routine group nursing.Nursing care of patients in routine group who received peritoneoscope to promote rapid recovery;the massage group were selected Weishu,Pishu,dachengshu,xiaochangshu,Zusanli and Shangjuxu points for massage;the ointment massage group was treated with Dachengqi Decoction ointment,and the selected points were the same as the massage group.The intervention began 24 hours after the operation,the intervention time was 7 days.The recovery time of bowel sounds,the first time of anal exhaust and the first time of defecation were compared.7 days after operation,GSRS was used to evaluate the feedback of gastrointestinal symptoms in the past 1 week.Abdominal pain anddistention were assessed by abdominal distention scale and VAS on the postoperative day 1(before intervention),2,3,4.On the 1st and 7th day after operation,the syndrome was scored with TCM syndrome score table of gastrointestinal diseases.The number of nausea,vomiting,infection,urinary retention and intestinal obstruction in the three groups during the intervention period were observed.After the patient is discharged from the hospital,the total cost of hospitalization are counted,the length of postoperative hospital stay.The collected data was input with Epi Data and analyzed with SPSS21.0.Results: There was no statistical difference in general data among the three groups(P > 0.05).The research data are balanced and comparable.(1)There was no statistical difference in the recovery time of bowel sounds among the three groups(P > 0.05).The first time of anal exhaust in the routine group,massage group and ointment massage group was65.17 ± 10.72 h,60.53 ± 11.80 h,53.22 ± 8.35 h,and the first time of defecation was 98.45 ± 22.43 h,82.13 ± 19.87 h,68.09 ± 20.53 h.The results showed that the first time of anal exhaust and defecation in the group of ointment massage was significantly shorter than that in the other two groups,the difference was statistically significant(P < 0.05).(2)The differences of abdominal pain and abdominal distention scores of three groups at different time points(the postoperative day 1,2,3,4)were statistically significant(P < 0.05).The effect of different intervention methods on reducing postoperative abdominal pain and abdominal distention scores in three intervention groups was evaluated as follows:ointment massage > massage group > routine group;the effect of four measurement times on reducing postoperative abdominal pain and abdominal distention scores was evaluated as follows: postoperative day4 > day 3 > day 2 > day 1(before intervention).(3)GSRS scores of ointment massage groups were lower than that in massage group and routine group,and the differences were statistically significant(P < 0.05).(4)The incidence of nausea and vomiting,infection,urinary retention and intestinal obstruction in three groups,ointment massage groups was lower than that in massage group and routine group,and the difference was statistically significant(P < 0.05).(5)The total effective rate of the ointment massage groups was 92.1%,higher than that of the massage group and the routine group,the difference was statistically significant(P< 0.05).(6)Compared with the total cost of hospitalization and the length of postoperative hospital stay.The results showed that the length of postoperative hospital stay of ointment massage group was shorter than the other two groups,the difference was statistically significant(P <0.05),the total cost of hospitalization of ointment massage group was lower than that in the massage group and the routine group,the difference was statistically significant(P < 0.05).Conclusion: Ointment massage can shorten the first time of anal exhaust and defecation,reduce the GSRS score and postoperative complications,and effectively promote the recovery of gastrointestinal function.At the same time,Ointment massage can effectively reduce abdominal pain and distention in patients with colorectal cancer after operation.Ointment massage can also shorten the length of postoperative hospital stay,to a certain extent,reduce the total cost of hospitalization.
