Objectives : To explore the influence of midwifer-led centralized group prenatal care on self-efficacy,prenatal anxiety, body quality management and outcome of delivery, and to provide a new prenatal care model to meet the needs of pregnant women for clinical reference. Methods:In this study,a randomized method was used to randomly divide104 primiparas who had undergone routine obstetrical check-ups in the obstetrics outpatient clinic of Ruikang Hospital affiliated to Guangxi University of Traditional Chinese Medicine. The pregnancy care model of the group intervened separately.Data collection and statistical analysis were performed on the body mass index, pregnant women's prenatal mental state, childbirth self-efficacy,and childbirth outcome before and after intervention.Statistical processing and analysis using SPSS20.0: descriptive statistical analysis of frequency, percentage, mean, standard deviation, etc.of the study object's age, occupation, education level, gestational week, etc.,counting data using chi-square test, measurement data An independent sample t test was used. The chi-square test is used for delivery methods. The time of labor,2hours postpartum hemorrhage, neonatal weight and 1 minute score, and body weight are first tested for normality. The t test must meet two conditions, which must meet the normal distribution and have the same variance Sex;if the data is non-normally distributed,rank-sum test is used, represented by the median and upper and lower quartiles. The anxiety score and self-efficacy score of pregnant women before and after intervention were tested by normal test, two independent sample t tests were used, and the paired t test was used in the group.Results:(1)Comparison of delivery outcomes: comparison of delivery methods: the cesarean section rate of the experimental group was 15%,and the cesarean section rate of the control group was 37%.The difference between the two groups was statistically significant(c2= 5.94,P=0.015 <0.05).,and the cesarean section rate of the experimental group was lower than that of the control group. There were statistically significant differences between the two groups in the second stage of labor, blood loss 2 hours postpartum, and newborn weight(P<0.05).The total labor process, the first labor process, the third labor process, and the Aparg score of 1 minute newborn were compared between the two groups, and the difference was not statistically significant(P>0.05).(2)Comparison of anxiety scale scores:before the intervention, there was no statistically significant difference between the anxiety scores of the experimental group and the control group(P>0.05),and statistically significant difference between the two groups after the intervention(t=4.14,P<0.05).The experimental group was superior to the control group in relieving anxiety.(3)Comparison of body mass score: the body mass index of pregnant women in the experimental group was significantly lower than that in the control group at admission after the implementation of the two interventions, and the difference was statistically significant(P<0.05).(4)Comparison on the scale of labor self-efficacy: before intervention, there was no statistically significant difference in anxiety scores between the experimental group and the control group(P>0.05).,after intervention, there was a statistically significant difference in labor self-efficacy between the two groups(P<0.05),and the experimental group was superior to the control group in terms of labor self-efficacy.Conclusion:(1)The intervention of a centralized group of pregnancy care led by midwives to the primiparas can reduce the antenatal anxiety of the primiparas,so as to improve the sense of self-efficacy of the primiparas and improve their coping ability during childbirth.(2)The centralized group of pregnancy health care led by midwives can intervene in the primary maternal, effectively control the body quality in the third trimester of pregnancy, shorten the total labor time, and reduce the cesarean section rate.
