OBJECTIVE:To investigate the clinical features of Clostridium difficile infection in patients with refractory inflammatory bowel disease,so as to provide dasis for clinical prevention and treatment of Clostridium difficile infection.METHODS:From Jan,2013 to Jan,2016,92 patients with active refractory inflammatory bowel disease and 21 cases of remissive refractory inflammatory bowel disease admitted to our hospital were continuously studied.The fluorescent enzyme immunoassay was used to detect the Clostridium difficile infection in the feces.And the clinical features of Clostridium difficile infection in patients with refractory inflammatory bowel disease were studied as well.RESULTS:A total of 19 cases of active refractory inflammatory bowel disease developed Clostridium difficile infection,and the infection rate was 20.65%.The infection rate of patients with ulcerative colitis was 20.83%,while it was 20.45%in patients with Crohn's disease.The usage of antibiotics,vancomycin and an active stage of the disease in patients with refractory inflammatory bowel disease got an elevated infection rate of Clostridium difficile(P<0.05).The infection rates of ulcerative colitis patients with severe,moderate and mild were 44.44%,5.56%and 8.33%,and the difference was statistically significant(P<0.05).Moreover,patients with colonic involvement got a higher incidence of Clostridium difficile infection(P<0.05).The rate of Clostridium difficile infection in colonic involvement patients with Crohn's disease was significantly higher(P<0.05).CONCLUSION:Clostridium difficile infection in patients with refractory inflammatory bowel disease is higher,and the antibiotic,vancomycin,disease activity,severity and colonic involvement are associated with Clostridium difficile infection.
