Objective: On the basis of the MRI-pathological control method,to investigate the correlation between the DCE-MRI quantitative parameters and HIF-1? or Ki-67 in the soft tissue sarcoma.Materials and Methods: 1.Case data: From March 2019 to January 2020,88 patients with STS suspected by clinical or imaging examinations were collected in a hospital,22 of whom were confirmed by pathology after operation,with a total of 24 lesions?one patient including 3 independent lesions?.24 cases of all STS contained leiomyosarcoma?5/24?,liposarcoma?6/24?,Dermatofibrosarcoma?2/24?,Myxoid fibrosarcoma?2/24?,Undifferentiated pleomorphic sarcoma?2/24?,synovial sarcoma?1/24?,Malignant giant cell tumor of tendon sheath?1/24?,malignant peripheral nerve sheath tumor?1/24?,angiosarcoma?1/24?,extraosseous myxoid chondrosarcoma?1/24?.There were 11 females and 11 males,aged 12-85 years.2.MR technology 1)MR equipment and technology: Using GE Discovery 750 w 3.0 T MR and 16 channel phased array abdomen coil,flexible coil,knee joint coil,wrist joint coil and shoulder joint coil,routine and enhanced MRI and DCE-MRI examinations were performed on the patients.2)Image post-processing and data acquisition: import the original DCE-MRI image to the GE-ADW 4.7 post-processing workstation,use the Gen IQ function software to obtain the quantitative parameter pseudo color images of Ktrans,Kep and Ve,check the region of interest in the most obvious tumor enhancement area,and obtain the quantitative parameter values.3.Comparison method between MRI and Pathology 1)Compare the image surface with the pathological section 2)Compare ROI of the image slice with the pathological sampling spot 3)Tissue section observation and HIF-1?,Ki-67 detection.4.Observation index 1)Conventional MRI manifestations: tumor location,size,shape,boundary,signal and enhancement.2)DCE-MRI quantitative parameter: Ktrans?Kep?Ve.3)Histopathological characteristics: cell morphology,structural characteristics,interstitial characteristics,nuclear atypia,mitotic image count,and necrotic area.4)HIF-1??Ki-67 expression 5.SPSS 21.0 software was used to analyze the data.P<0.05 was considered statistically significant.If the counting data were normally distributed,Pearson test was performed for the correlation analysis of Ktrans value,Kep value and Ve value with HIF-1? and Ki-67 expression,respectively.If the counting data were nonnormal distribution,Spearman test was performed on the correlation analysis.Results: 1.The MRI findings of 24 cases of STS 1.1 Conventional MRI manifestations: The tumors were mostly located in the extremities,including 10 in the thigh,4 in the calves and shoulders,1 in the head,hip,back,hand,foot and chest wall,1 in the head,9 in the back,9 in the chest wall,15 in the maximum diameter of the tumor larger than 5cm,16 in the blurry boundary,8 in the clear boundary,21 in the T1 WI,3 in the slightly high signal,20 in the T2 WI,4 in the high and low mixed signal,18 in the uneven enhancement Cases,6 cases with homogeneous enhancement.1.2 STS DCE-MRI manifestations DCE-MRI findings of soft tissue sarcoma: the solid components of 24 cases of soft tissue sarcoma showed obvious continuous enhancement.The false color images of Ktrans,kep and ve showed different color scales from blue to red.The lesions were obviously enhanced in the pseudo color image and showed as red high perfusion area,moderate enhancement showed as yellow and green medium perfusion area,and slight enhancement or no enhancement showed as blue low perfusion or no perfusion area.1.3 STS DCE-MRI parameters The quantitative parameters of DCE-MRI of 24 soft tissue sarcomas were Ktrans,kep and VE,which were normal distribution.The range of Ktrans was 0.29-2.00/min,the average was 1.03 ± 0.53/min;the range of kep was 0.40-2.67/min,the average was 1.40 ± 0.74/min;the range of VE was 0.41 ± 0.99,the average was 0.80 ± 0.14.2.The pathological observation of 24 cases of STSs: The mean HIF-1? expression was 6.17±2.82,and the mean Ki-67 expression was 27.7%±25%.3.The correlation between DCE-MRI parameters and HIF-1? or Ki-67 in the STS 1)The correlation between DCE-MRI parameters and HIF-1? expression STS DCE-MRI quantitative parameter values Ktrans and Kep values were significantly positively correlated with HIF-1? expression,and had statistical significance?P <0.05?;Ve values was not correlated with HIF-1? expression.2)The correlation between DCE-MRI parameters and Ki-67 expression STS DCE-MRI quantitative parameters Ktrans,Kep,Ve values have no correlation with the soft tissue sarcoma Ki-67 expression.Conclusions: On the basis of MRI-pathological comparison,correlation analysis of DCE-MRI parameters with HIF-1? and Ki-67 in 24 STS was conducted,and the following conclusions can be drawn: 3.The STS DCE-MRI quantitative parameters Ktrans and Kep are correlated and positively correlated with HIF-1?,that is,as the Ktrans and Kep values increase,the pathological HIF-1? expression is higher,and vice versa.The Ve value has no correlation with its HIF-1? expression.4.The STS DCE-MRI quantitative parameters Ktrans,Kep,Ve have no correlation with Ki-67.
