Objective:To explore the occurrence and influencing factors of severe intraventricular hemorrhage(IVH)in extremely premature infants.Methods:The clinical data of neonates with a gestational age of less than 32 weeks in the NICU of Hunan Children's Hospital from January 2016to April 2019 were collected retrospectively and were divided into severe IVH group and non-severe IVH group according to the severity of intraventricular hemorrhage.The prenatal,antenatal,and postnatal factors were compared between the two groups.Through single factor analysis and multivariate logistic regression analysis,the independent risk factors affecting severe IVH in extremely premature infants were discussed.Results:1.A total of 448 extremely premature infants were included in this study.54 cases of severe IVH occurred,and the incidence rate was 12.1%.In severe IVH,33 cases(61.1%)had a gestational age of<28 weeks and 13 cases had a gestational age of 28-29~+66 weeks(24.1%),8 cases (14.8%)with gestational age 30~31~+66 weeks.2.The clinical data of infants in severe IVH group and non-severe IVH group were compared.The results showed that the severe IVH group had a gestational age of<28 weeks,early-onset sepsis,intubation resuscitation in the delivery room,neonatal respiratory distress syndrome,hypercapnia,and mechanical ventilation within 3 days after birth were higher than the non-severe IVH group.The difference was statistically significant(P<0.05).3.Multivariate logistic regression analysis was conducted on factors with statistically significant differences in single factor analysis(P<0.05),and it was found that gestational age<28 weeks and early-onset sepsis were independent risk factors for extremely premature infants with severe IVH.Conclusions:1.The incidence of severe IVH is very high in extremely premature infants.2.Gestational age<28 weeks and early-onset sepsis are independent risk factors for severe IVH in extremely premature infants.
