Objective: Breast cancer is a kind of highly hormone dependent cancer,in which estrogen receptor(ER)and progesterone receptor(PR)are involved in the occurrence and development of breast cancer,which has been confirmed.Many endocrine therapies have been used to treat ER positive breast cancer.However,with the increase of endocrine resistance in patients,the local recurrence or distant metastasis of the tumor will eventually lead to new challenges in the treatment of ER positive breast cancer.It is urgent to actively seek new targets for anti-tumor treatment and restore endocrine sensitivity.Androgen receptor(AR)is a member of steroid receptor family,but it is still controversial whether AR can be used as a new anti-tumor therapeutic target and prognostic factor for breast cancer.From January 2012 to December 2012,the clinicopathological data of ER positive female primary breast cancer patients in the Affiliated Hospital of Southwest Medical University were sorted out and followed up,so as to analyze the relationship between the expression of AR and the clinicopathological characteristics of ER positive breast cancer patients and the impact on the prognosis of ER positive breast cancer patients,in order to look for new ER positive breast cancer patients Objective: To explore a new treatment for ER positive breast cancer patients.Methods: From January 2012 to December 2012,ER positive female primary breast cancer patients who were treated in breast surgery of the Affiliated Hospital of Southwest Medical University were selected as the study objects,among which 130 cases had complete clinicopathological data and met the inclusion criteria.The follow-up methods were telephone follow-up and outpatient follow-up.The deadline for follow-up is December 2018.The definition of this study: total survival time(OS)refers to the period from the first day after operation to the date of death or last follow-up.Tumor free survival period(DFS)refers to one of the following conditions from the first day after operation to the local recurrence or distant metastasis of the patient:(1)(2)(3)new contralateral breast cancer;new other malignant tumor;death or loss of visit of the patient.The treatment methods are: operation and / or chemotherapy and / or endocrine therapy and / or local radiotherapy.The expression of AR protein in ER positive breast cancer tissues was detected by spss25.0 software.Meanwhile,survival analysis was carried out by Kaplan Meier method.The expression of AR protein was compared with age,tumor diameter,invasive biological behavior,molecular typing and other clinicopathological characteristics of patients and survival after operation Situation.Results: 1.Expression of ARAmong 130 ER positive breast cancer patients,76(58.5%)had AR protein positive expression.2.Relationship between AR expression and clinicopathological features of ER positive breast cancerThe positive rates of AR protein expression in patients with low differentiation,clinical stage ? + ?,p53 positive,neurovascular invasion and lymph node metastasis were significantly lower than those in patients with medium differentiation and high differentiation,clinical stage ? + ?,p53 negative,no neurovascular invasion and no lymph node metastasis,and the difference was statistically significant(P < 0.050).The positive rate of AR protein expression was not related to age,menstrual state,tumor diameter,progesterone receptor,HER-2 state and Ki-67.3.Relationship between AR expression and prognosis of ER positive breast cancer patientsThe 3-year and 5-year overall survival and tumor free survival of AR positive patients were significantly higher than those of AR negative patients,and the difference was statistically significant(P < 0.050).At the same time,the 5-year cumulative total survival curve and tumor free survival curve of AR protein positive patients were significantly better than those of AR protein negative patients,and the difference was statistically significant(? 2 = 8.134,P = 0.004;? 2 = 9.150,P = 0.002).Conclusion: 1.In ER positive breast cancer patients,AR protein expression rate was higher.2.AR protein positive expression in ER positive breast cancer has better tumor differentiation,lower clinical stage and weaker tumor invasiveness.3.After standardized treatment,the long-term survival of patients with AR protein positive expression is also better than that of patients with AR protein negative expression,suggesting that AR may become a potential new target for ER positive breast cancer treatment,and may be one of the prognostic factors.
