Study On The Effect Of Zhilong Huoxue Tongyu Capsule On Immune Function In Patients With Type Wind Phlegm & Blood Stasis Resistance Acute Ischemic Stroke

Abstract/Summary:
Objective: 1.To investigate the changes of CD3+,CD4+,CD8+T cells,TNF-α,IL-1β and IL-6 cytokines in peripheral blood of patients with Acute ischemic stroke.2.To understand the status of the above immune cells and cytokines in patients with different degrees of cerebral infarction.3.To observe the dynamic changes of The above immune cells and cytokines,The National Institutes of Health Stroke Scale and The Barthel index in AIS patients.By evaluating the effect of Zhilong Huoxue Tongyu Capsule on the above indicators and the clinical effect evaluation,the mechanism of action was preliminarily discussed.Methods: 1.The subjects were 60 patients with acute ischemic stroke(within 24 h of onset)who were admitted to the Department of Cardioencephalopathy,Traditional Chinese Medicine Hospital Attached to Southwest Medical University from October 2018 to August 2019,and were identified by two deputy director traditional Chinese medicine physicians who were of type wind phlegm & blood stasis resistance based on syndrome differentiation as the case group(stroke group).A control group of 20 healthy patients with one or more susceptibility factors to cerebrovascular diseases in the physical examination center and age and gender matching the case group(stroke group)were established.According to NIHSS score,60 cases were divided into mild stroke group,moderate stroke group and severe stroke group.At the same time,the patients were randomly divided into Zhilong group and basic group by random number table method.2.Basic treatment(including secondary prevention,routine treatment and rehabilitation treatment)was given to both the Zhilong group and the basic group.In the Zhilong group,1.6g of Zhilong Huoxue Tongyu Capsule was added to the basic treatment three times a day,and the treatment course was observed for 1 week.3.Whole blood CD3+,CD4+ and CD8+T cells were detected by ELISA,serum TNF-α was detected by radioimmunoassay,and serum IL-1β and IL-6 cytokines were detected by chemiluminescence.The NIHSS score and BI score were evaluated by the same trained physician.4.The results were recorded in EXCEL,and SPSS 21.0 was used for statistical analysis.Results: 1.Compared with the control group,CD3+,CD4+ and CD8+T cells in the peripheral blood of the stroke group were lower than that of the control group,while TNF-α,IL-1β and IL-6 levels were higher than that of the control group,with statistically significant differences(P<0.05).2.In mild stroke group,moderate stroke group and severe stroke group,peripheral blood CD3+,CD4+ and CD8+T cells gradually decreased,while TNF-α,IL-1β and IL-6 gradually increased.The difference between CD3+ and CD4+T cells was statistically significant(P<0.05).The difference between TNF-α and IL-6 mild group,moderate group and severe group was statistically significant(P<0.05).3.From day 1 to day 7,CD3+,CD4+,and CD8+T cells in the whole blood of patients in the Zhilong group and the basic group were the lowest on day 3,and increased on day 7.The changes of CD3+,CD4+ and CD8+T cells in the whole blood of the Zhilong group were statistically significant(P<0.05),and the changes were more significant than those in the basic group(P<0.05).4.From day 1 to day 7,the serum TNF-α,IL-1β and IL-6 levels of patients in both the Zhilong group and the basic decreased gradually.The serum levels of TNF-α,IL-1β and IL-6 in the Zhilong group were significantly decreased(P<0.05),and were significantly decreased compared with the basic group(P<0.05).5.After treatment,the NIHSS score of the Zhilong group and the basic group decreased,while the Barthel score increased(P<0.05).In the 7th day,the NIHSS score of the Zhilong group was significantly lower than that of the basic group,and the BI score was significantly higher than that of the basic group,with statistically significant differences(P<0.05).Conclusion: 1.The severity of acute ischemic stroke was negatively correlated with CD3+,CD4+,and CD8+T cells,and positively correlated with TNF-α,IL-1β and IL-6.After stroke 1 to 7 days,CD3+,CD4+,and CD8+T cells in patients’ peripheral blood showed a trend of decreasing first and then increasing,while TNF-α,IL-1β and IL-6 gradually decreased.2.Zhilong Huoxue Tongyu Capsule can promote immunosuppression after stroke and has a protective effect on brain tissues of AIS patients.3.Zhilong Huoxue Tongyu Capsule can promote the recovery of neurological function in AIS patients,improve the prognosis of the patients and improve their ability of daily life.
