| Objective:To investigate the correlation between peripheral blood neutrophil/lymphocyte ratio(NLR),platelet/lymphocyte ratio(PLR)and prognosis in patients with small cell lung cancer(SCLC).Methods: This study included 150 patients with SCLC in Respiratory Medicine Department,Yijishan Hospital from July 2015 to July 2017.The clinical data such as gender,age,smoking history,EOCG score,treatment method,disease stage,NSE value,anemia and peripheral blood neutrophil,platelet and lymphocyte count were collected at the time of first diagnosis.And referring to the related literature,the best cut-off value of NLR was decided to be 2.86 and the best cut-off value of PLR was decided to be 191.According to two optimal cut-off values,all SCLC patients were divided into high and low NLR groups,high and low PLR groups.Software SPSS 25.0 was used to analyze the clinical characteristics of different NLR,PLR groups and the survival risk factors of patients with SCLC.After that,the survival difference was analyzed by Log-rand method,and all the results were evaluated comprehensively to find out the significance of NLR,PLR in the prognosis of patients with SCLC.Results: A total of 150 patients were included in this study,including 103 males and 47 females(2.20:1);88 smokers and 62 non-smokers(1.42:1);78 cases of low ECOG score(0-1),72 cases of high ECOG score(≥ 2)(1.10:1);34 cases received thoracic radiotherapy and 116 cases did not receive thoracic radiotherapy(0.29:1);115 cases treated with chemotherapeutic drugs,35 cases not treated with chemotherapeutic drugs(3.29:1);57 cases with NSE < 32.6ng/l,93 cases with NSE ≥ 32.6ng/l(0.61:1);54 cases with limited period SCLC,96 cases with extensive SCLC(0.56:1).According to the cut-off values of 191 and 2.86 obtained by consulting the relevant literature,all patients were divided into high or low NLR group and high or low PLR group.According to the basic clinical data of the patients,the results showed that there was significant difference between high or low PLR group and high or low NLR group on whether they were treated with chemotherapy drug(P<0.05),while the comparison of other clinical characteristics showed no significant difference(P>0.05).In terms of survival analysis,NLR and PLR had a significant effect on survival.The survival time of low NLR was significantly longer than that of high NLR(20 months:13.2 months,P<0.05).The survival time of low PLR was significantly longer than that of high PLR(20.3 months:12 months,P<0.001).Multivariate analysis showed that NLR and PLR were independent risk factors for the prognosis of SCLC.(HR: 2.00,95%CI: 1.20~3.34,P=0.008;HR: 1.63,95%CI: 1.05~2.25,P=0.031).Conclusion: Through this study,it can be concluded that NLR and PLR before treatment may be independent risk factors for predicting the prognosis of patients with SCLC.The prognosis of patients with high value of NLR,PLR at the time of first diagnosis was also relatively poor.Therefore,NLR and PLR have important value in evaluating the prognosis of SCLC. |
