Objective:1.To understand the current status of nursing staffing and clinical nursing work in the nursing homes and to calculate the nursing human resources allocation in the nursing homes based on workload measurement and the ability level of the elderly.2.Establish a calculation model of nursing staffing in the nursing homes in terms of quantity and structure,in order to save the labor costs of the nursing homes,and to allocate and use nursing manpower reasonably and effectively,and to provide reference and guidance for the long-term development of high-quality nursing services.Methods:1.Through the literature review and the discussion method of the research group,the elderly capacity assessment scale for the aged care institutions issued by the Ministry of Civil Affairs was screened.The elderly can be divided into four categories:intact capacity,mild disability,moderate disability,and severe disability.2.In 4 demonstration nursing homes in Jiangsu Province,we used field investigation to measure nursing man-hours;We used questionnaires to investigate general information about nursing staff in institutions and conduct capacity assessments for the elderly.3.Based on the actual workload data of different nursing positions,the recommended allocation ratio of nursing staff in the nursing homes for 24h is obtained through the method of expert correspondence.4.Based on the ability level of the elderly,the number of nursing hours,and the recommended staffing ratio,the wide release rate and mobility coefficient required for manpower deployment are cited to establish a nursing manpower allocation model for the nursing homes.Results:1.General information of nursing staff and senior citizens in four nursing homes:Nurses have low seniority,low work experience,and low professional titles.Nurses under the age of 30 have reached 64.1%,with college education or above account for 89.1%.Elderly caregivers are generally older(60.1%between the ages of 50-59),low in education(91.1%for junior high school and below),and a high proportion of people with high mobility and without employment(53.1%).A total of 938 elderly people were admitted.Most of the elderly were over 80 years old,48.6%were severely disabled.The elderly had a higher education level,with 40.9%of high school education or above.2.The average 24h nursing man-hours for the elderly:a total of 938 elderly people,of which the average length of direct care for the elderly with intact capacity,mild disability,moderate disability,and severe disability was 8.17mins,18.29mins,53.44mins,87.57mins' the difference was statistically significant(P<0.001).3.24h nursing man hours for different nursing positions:care workers,duty nurses,and nursing managers provide 46251.6mins(88.9%),4968.5mins(9.5%),832.1mins(1.6%)of direct nursing hours to the elderly in 24h.4.Average daily indirect nursing hours:Word processing(33%)and drug management(21%)account for half of the indirect nursing services.Each elderly person's indirect nursing service is about 7.92 minutes per day.5.Percentage of total hours worked per day for different nursing posts:The total hours worked for care workers,duty nurses,and nursing managers are 55.69%,22.95%,and 21.36%,respectively.The average length of care(HPRD)served to each elderly person per day was 50.86mins,9.58mins,and 2.97mins in that order.6.Nursing staff allocation model in the elderly care institution:The total number of nursing staff in the 24h is(A*8.17+B*18.29+C*53.44+D*87.57+N(ABCD)*7.92)/480× 1.23×1.46,among which the positions of care workers,duty nurses,and nursing managers accounted for 64%,23%,and 13%respectively;of which,A,B,C,and D were in order of intact capacity,mild disability,moderate disability,and severe disability;NABCD is the total number of seniors.Conclusion:1.At present,although the service provision of the elderly care institutions includes the contents of "medical" and "care",daily care is still the focus of service.Other types of care services are too singular,especially lacking communication and spirituality consolation,health management.2.Due to the shortage of nursing manpower in nursing homes,structural imbalances and inadequate qualifications,the workload of elderly care workers is large,and the average length of care for elderly people is low.3.It is necessary to improve the direct nursing time of the elderly in the nursing homes to improve the overall nursing service quality of the institutions,and gradually establish and improve the old-age care model with elderly rehabilitation as its core.4.Based on the nursing workload and the ability level of the elderly,combined with expert opinions.We have initially constructed a nursing manpower allocation model for nursing homes,to provide a theoretical basis for manpower allocation by managers,and to promote standardized management of nursing homes.
