Objective:To explore the therapeutic effects of auricular magnetic bead therapy and Kaitianmen combined therapy on sleep quality and fatigue.To analyze the effectiveness and safety for improving the mental health and quality of life of the patients after stroke in the community.In order to summarize the high-quality traditional Chinese medicine nursing program for the patients after stroke in the community.To provide new ideas for future clinical intervention.Methods:In this trial,60 community stroke patients who met the inclusion criteria were selected.We randomly divided the patients into three groups,including the auricular magnetic bead group(20 cases),the Kaitianmen group(20 cases)and the combined group(20 cases).The auricular magnetic bead group applied auricular magnetic bead therapy based on the conventional nursing.The Kaitianmen group applied Kaitianmen combined therapy based on the conventional nursing,and the combined group applied auricular magnetic bead therapy and Kaitianmen combined therapy based on the conventional nursing.The Pittsburgh Sleep Quality Index(PSQI)was used to evaluate the patient's sleep status before and after intervention;The Chalder Fatigue Scale(CFS)was used to evaluate the fatigue degree of patients before and after the intervention;The 10-Item Kessler Psychological Distress Scale(K10)was used to evaluate the mental health of patients before and after intervention;The EuroQol Five-dimension Questionnaire(EQ-5D)was used to evaluate the quality of life before and after intervention.Result:1.Before the intervention:The baseline comparison of the demographic data of gender,age,marital status,education level,etc;The clinical data of stroke diagnosis category,stroke duration,complications,etc;The efficacy indicators of sleep quality,fatigue,mental health status,and quality of life of the three groups of subjects were no difference before the intervention(P>0.05).2.After the intervention:After intervention,Comparison between groups of the total scores of the sleep scale and the mental health scale of three groups of patients were all different(P<0.05).Among them,the combined group had the best effect on sleep quality than the other two groups,and had better effect on mental health than the auricular magnetic bead group.Comparison between groups of the fatigue scale,the quality of life scale score of three groups of patients were no difference(P>0.05).Comparison of the total scores of the sleep scale of three groups of subjects before and after the intervention were different(P<0.05).The specific analysis found that the intra-groups comparison of several factors including the sleep quality,time to sleep,sleep time,sleep efficiency,sleep disorders,and daytime dysfunction were different(P<0.05).The combined group had the best effect.Comparison of the total scores of the fatigue scale of three groups of subjects before and after the intervention were different(P<0.05).The specific analysis found that the physical factor and mental factor of the three groups before and after the intervention were different(P<0.05),and the combined group had the best effect.Comparison of the total scores of the mental health scale,the quality of life scale of the three groups of subjects before and after the intervention were different(P<0.05).Conclusion:Comprehensive analysis of the three intervention methods can effectively adjust the sleep status and reduce fatigue,thereby improving the mental health and quality of life of the patients after stroke in the community.In addition,auricular magnetic bead therapy combined with Kaitianmen combined therapy had the best effect on clinical symptoms of patients after stroke in the community than auricular magnetic bead therapy or Kaitianmen combined therapy.The intervention methods of this study are cost-effective,simple,and easy to implement.They provide a reference for the sleep and fatigue of stroke patients in the comprehensive care community in the future.
