| Research background IgA nephropathy is the most common primary glomerular disease in China,which mainly occurs in young and middle-aged men.Currently,most studies believe that this disease is chronic and progressive.About 1/3 of the patients show renal insufficiency,and about 20% to 30% of the patients will develop into end-stage kidney disease(ESRD)within 10 to 20 years,which is one of the most common causes of ESRD in China[1].Clinical with recurrent hematuria as the main feature,its pathogenesis is complex,has not yet been clear,so there is nounified and effective method of western medicine treatment.In recent years,a large number of literature from the perspective of traditional Chinese medicine study of IgA nephropathy,found its attributable to "traces" "drowned" blood "Sou blood" category,such as the pathogenesis characteristics of the virtual to real,lung spleen kidney empty as this,butthe real in hot and humid in pathogenic most see more,more "hardness of disease,to the hot and humid," says,a variety of experimental and clinical studies have confirmed the basic characteristics of damp and hot is injured kidney nephritis pathogenesis,therefore been cooling method in the treatment of IgA nephropathy with blood,and offers a new way for the clinical treatment of IgA nephropathy.Objective to investigate the clinical effect of clearing heat,dampening and cooling blood on IgA nephropathy.Methods 60 patients who met the inclusion criteria were selected and divided into treatment group(30 cases)and control group(30 cases)by the method of lottery sampling.Both groups were given the basic treatment of general western medicine,andthe treatment group was treated with the prescription of clearing heat and dampness and cooling blood(Jiaweixiaojiyinzi).The laboratory indexes after 4 weeks and 8 weeks of treatment and the syndrome of dampness and heat after 8 weeks of treatment were observed respectively.The laboratory indexes mainly included 24-hour albuminuria quantification,urinary albumin creatinine ratio,urinary red blood cell count,renal function(Scr,BUN,GFR),plasma albumin,blood lipid(TG,CHOL),and plasma c-reactive protein.SPSS22.0 software was used for medical statistical analysis of experimental data to evaluate the efficacy.Results During the study,no significant adverse reactions were found in the two groups,and all the selected cases were included.1.After treatment,the laboratory indexes of the two groups were compared.The differences in 24 h proteinuria,urinary proteincreatinine ratio,urinary red blood cell count,plasma albumin,blood lipid(TG,CHOL)and CRP were all statistically significant(P<0.05).The differences in renal function(Scr,BUN,e-gfr)were not statistically significant(P>0.05).2.The difference of TCM syndrome scores between the two groups after treatment was statistically significant(P<0.05).For the single symptoms(skin swelling,sore choice,sore throat,urine yellow,hot or astringent pain adverse,face or limbswelling,bitter or dry mouth,mouth sticky,wan stuffy stay,dry mouth do not want to drink),the difference has statistical significance(P<0.05).3.The total effective rate of TCM treatment in the treatment group and control group was 93.3% and 76.7%,respectively,with statistically significant differences(P<0.05).The total effective rate of western medicine was 90% and 73.3%,respectively,with statistically significant differences(P<0.05).4.In the whole treatment process,there was no statistical difference between the treatment group and the control groupin blood routine,liver and kidney function,electrolyte and other safety indicators before and after treatment(P>0.05).Conclusion Heat-clearing,moisture-cooling and blood-cooling mainly treat patients with damp-heat IgA nephropathy,which can effectively reduce proteinuria,hematuria,reduce blood lipid,increase plasma albumin,reduce inflammatory reaction,stabilize renal function,and significantly improve patients' symptoms of traditional Chinese medicine,and drug safety. |
