Efficacy And Safty Of Sofren Injection On Acute Coronary Syndrome A Meta Analysis

Objective:The meta-analysis is to appraise the availability of So fren injection on angina pectoris,ECG's availability,Hemorheolog y[whole blood viscosity(high shear blood viscosity,low shear blo-od viscosity)?plasma viscosity,fibrinogen],cardiac function(pro NT-BNP,LVEF),Inflammatory markers(hs-CRP?IL-6?TNF-?),lipid lev-els(TG?TC?LDL-C),antioxidant index(SOD)?platelet function(p-latelet aggregation rate,TXB2,PAI-1)and adverse reactions in patients with ACS.Methods:The randomized controlled trial of Sofren injection on Ac ute Coronary Syndrome published From the date of construction to February 2020 was searched by computer,The databases include: CNK I,VIP,Wanfang,Sinomed,Pub Med,CBM and Web of science.The key words of which including:“Da Zhu Hongjingtian injection”,“Sofren injec-tion ”,“coronary heart disease”,“acute coronary syndrome”,“acute myocardial infarction”,“acute ST segment elevation myoca-rdial infarction”,“acute non ST segment elevation myocardial i-nfarction”,“unstable angina”,“chest obstruction”,“random-ized controlled trial” were serched in Chinese and English.All data were analyzed by Revman 5.3.Resultes:There were 663 literatures,51 of which met the requirem-ents,4419 patients.Compared with the Control Group,the Treatm ent Group of Sofren injection is beneficial to improve the clinic al effect of angina pectoris,the actovoty ratio(RR=1.43,95%CI:1.33~1.55,P<0.00001)and overall response rate(RR=3.76,95%CI:3.06~4.762,P<0.00001)are better than Control Group;Improve electroc-ardiographic effect,the actovoty ratio(RR=1.51,95%CI:1.31~1.73,P<0.00001)and overall response rate(RR=1.24,95%CI:1.12~1.38,P<0.0001)aslo are better than Control Group;Decrease patients' CK-MB level(WMD=-5.61,95%CI:-9.14~-2.08,P=0.002);Decrease patie-nts' NT-pro BNP level(WMD=-539.22,95%CI:-730.00~-348.44,P<0.00001);Improve patients' LVEF level(WMD=7.18,95%CI:5.73~8.63,P<0.00001);Decrease patients' IL-6(WMD=-16.59,95%CI:-19.94~-13.25,P<0.00001)?hs-CRP(WMD=-5.16,95%CI:-6.21~-4.12,P<0.00001)and TNF-a level(WMD=-29.57,95%CI:-51.74~-7.40,P=0.0009);Redu-ce the whole blood viscosity of patients(WMD=-1.10,95%CI:-1.41~-0.80,P<0.00001)?fibrinogen(WMD=-0.62,95%CI:-0.75~-0.49,P<0.00001)and plasma viscosity(WMD=-0.25,95%CI:-0.33~-0.17,P<0.00001)level;Inhibit platelet aggregation(WMD=-5.98,95%CI:-6.46~-5.50,P<0.00001);Decrease the level of patients' TXB2(WMD=-14.24,95%CI:-18.02~-10.46,P<0.00001)and PAI-1(WMD=-14.76,95%CI:-27.55~-1.97,P=0.02);Reduce the level of patients' LDL-C(WMD=-0.41,95%CI:-0.55~-0.26,P<0.00001)?TC(WMD=-0.60,95%CI:-0.78~-0.42,P<0.00001)and TG(WMD=-0.18,95%CI:-0.24~-0.13,P<0.00001);Improve the level of patients' SOD(WMD=9.31,95%CI:6.24~12.38,P<0.00001).Nineteen literatures mentioned adverse reactions,seven had adverse reactions,and adverse reactions occurred in both gr oups.Conclusion: On the basis of routine treatment,Adding Sofren injec-tion,which can improve ACS patients' clinical effect of angina p-ectoris and ECG efficiency;Improve patient LVEF;Decrease the lev-el of patients' CK-MB,the whole blood viscosity(low shear bloo-d viscosity,high shear blood viscosity),plasma viscosity,fibrino-gen,NT pro BNP,inflammation index(hs-CRP,IL-6,TNF-?),blood lipi-d(TG,TC,LDL-C),SOD,TXB2,PAI-1 and Inhibit platelet aggregatio-n beter.Wihch can enhance the effective treatment rate and clini-cal cure rate of ACS patients,prevent the occurrence of postoper-ative complications of ACS,and improve the prognosis of patients.According to the existing literature research,it is believed that Sofren injection has definite curative effect and lessadverse rea-ctions in the treatment of ACS patients,which deserves clinical popularize.However,in the future clinical research,it is necessar-y to carry out more large sample and more high-quality RCT to be-ar out the efficacy and safety of Sofren injection in the treatm-ent of ACS.
