Objective:The aim of the study was to detect the serum expression levels of fibroblast growth factor-21(FGF-21)in fatty pancreas(FP)patients and to investigate their potential clinical value.Method:We selected 99 patients with fatty pancreas as the fatty pancreas(FP)group and 19 healthy volunteers as healthy control group(HC group).According to whether fatty pancreas patients with common metabolic diseases were divided into simple fatty pancreas group(Simple FP group,N=36),fatty pancreas group with fatty liver group(FP and FL group,N=25),fatty pancreas group with diabetes mellitus group(FP and DM group,N=24),fatty pancreas group with obesity group(FP and OB group,N=14).The t test and ?2 test were used for the preliminary statistical analysis of continuous variables and categorical variables respectively.The basic data,biochemical indexes,serum expression levels of FGF-21 and serum expression levels of four kinds of adipocytokines(Adiponectin,Leptin,Resistin,Tumor necrosis factor-alpha(TNF-?))were compared among different groups.And the correlation between serum expression FGF-21 levels and clinical biochemical indexes and serum inflammatory factors was analyzed.In addition,the logistic regression analysis was used to identify independent factors associated with fatty pancreas.A receiver operating characteristic(ROC)curve was used to evaluate the predictive value of serum FGF-21 for fatty pancreas patients.Results:1.Compared with the healthy control(HC)group,the age,weight,body mass index(BMI),waist circumference(WC),fasting blood glucose level,serum uric acid level,serum cholesterol level and serum triglyceride level of the fatty pancreas(FP)group were significantly higher,while the serum total bilirubin and high-density lipoprotein level were lower(all P<0.05).In addition,the prevalence of fatty liver,diabetes mellitus,hypertension,obesity,dyslipidemia in the fatty pancreas group was significantly higher than that in the HC group(all P<0.05).2.The serum expression levels of FGF-21,resistin and leptin in the fatty pancreas(FP)group were significantly higher than those in the healthy control(HC)group,while the serum expression levels of adiponectin were lower,and the differences were statistically significant(all P<0.05).The level of serum expression TNF-? in FP group was higher than that in HC group,but there was no statistical significance3.Pearson analysis showed that serum expression FGF-21 levels in FP patients were negatively correlated with serum expression leptin and cholesterol levels(r=-0.323,P=0.001;r=-0.205,P=0.042).4.The ROC curve showed that the best critical value of the serum expression FGF-21 level in FP patients was 187.85 pg/mL(AUC 0.716,P=0.002,95%CI 0.611-0.820).5.Multivariate logistic regression analysis showed that body mass index(BMI),serum expression FGF-21 level and serum high-density lipoprotein level were independent risk factors of fatty pancreas,with odds ratio(OR)of 2.210(95%CI 1.404-3.478),1.066(95%CI 1.001-1.012),0.030(95%CI 0.002-0.556).Conclusion:The level of serum FGF-21 in patients with fatty pancreas was significantly higher than that in healthy controls.The detection of serum FGF-21 level may help identify the susceptible population of fatty pancreas.
