Objective To evaluate the intervention effect of Taijiquan on insomnia in the elderly and explore a new way to treat insomnia in the elderly. Methods 93 elderly patients with insomnia were randomly divided into intervention group(n = 47)and control group(n = 46).The intervention group received two weeks of Taijiquan training, followed by a 24 week intervention experiment under the supervision of teachers;the control group received cognitive behavioral therapy. Before and after the intervention, the elderly in the two groups were collected for three times, including Pittsburgh sleep quality index(PSQI),Geriatric Depression Scale(GDS)and simple mental state examination scale(MMSE).Results1.After 24 weeks of intervention,65 cases(29 cases in the intervention group and 36 cases in the control group)of effective subjects were collected. There were 28 patients with loss, and the loss rate was30.10%.There was no significant difference between the two groups(P >0.05).There was no significant difference between the two groups(P >0.05).2.To evaluate the efficacy of sleep quality index score reduction rate, intra group comparison: after 12 weeks and 24 weeks of intervention, the difference in the total effective rate was statistically significant(P < 0.01).Comparison between groups: after 12 weeks of intervention, the difference of total effective rate between the two groups was statistically significant(P < 0.05);after 24 weeks of intervention, the difference of total effective rate between the two groups was not statistically significant(P > 0.05).3.Repeated measurement and analysis of PSOI results: there was no Statistical significance in the interaction of group x time(F = 2.577,P = 0.084);there was no statistical significance in the main effect of Taijiquan intervention on sleep improvement of subjects(F = 2.145,P =0.105);the time factor had statistical significance on sleep improvement of subjects(F = 0.874,P = 0.000).The difference between the two groups was statistically significant(P < 0.05).4.Repeated measurement and analysis of GDS results of subjects: there was nostatistical significance in the interaction of group x time(F = 1.063,P = 0.429);there was no statistical significance in the main effect of Taijiquan intervention on GDS of subjects(F = 1.209,P = 0.076);there was statistical significance in the effect of time factor on GDS of subjects(F = 0.561,P = 0.000).The difference between the two groups was statistically significant(P < 0.05).5.Repeated measurement and analysis of MMSE results: the interaction of group x time was statistically significant(F = 0.305,P = 0.009);the main effect of Taijiquan intervention on cognitive function was not statistically significant(F = 5.411,P = 0.040);the influence of time factor on cognitive function was statistically significant(F = 0.584,P = 0.000).The difference between the two groups was statistically significant(P < 0.05).Conclusion1.Taijiquan has a significant effect on insomnia in the elderly, which long-term effect is no worse than CBT-I recommended in the current guidelines.2.Taijiquan intervention can significantly improve the level of depression and cognitive function of the elderly, and the improvement of cognitive function is more significant with the extension of intervention time, and the long-term benefit is better than CBT-I.
