Objective:To investigate the epidemiological data of diabetic patients who come from community, analyze the risk factors,TCM syndromes and distribution characteristics, explore the correlation between syndrome types and relevant clinical data, provide a basis for the prevention and treatment of diabetic peripheral neuropathy. Methods:Select three health service center in HeFei,according to the diagnostic criteria for diabetes, the investigation object is divided into DPN with DPN group general information acquisition on the basis of case selection criteria, biochemical index to check including fasting plasma glucose(FPG),glycosylated hemoglobin(HbAlc)and postprandial 2 hours blood glucose(2 HPG),blood lipid indexes including total cholesterol(TC),triglyceride(TG),high-density lipoprotein(HDL-C)and low density lipoprotein(LDL-C),Liver function included AST and ALT,renal index creatinine(CREA)and uric acid(UA),TCM syndrome scale investigation, multivariate logistic analysis of risk factors for DPN,TCM syndrome distribution characteristics of DPN patients and its correlation with clinical indicatorsResults:We investigated 421 patients with diabetes,198 patients with DPN and 223 patients without DPN,The incidence was 47.03%,the 50 patients with mild DPN(25.25%),96 patients with moderate DPN(48.48%),and 52 patients with severe DPN(26.26%)1 FeaturesCompared to patients who without DPN,the patients with DPN their age, duration of diabetes,BMI,HbAlc 2hPBG and drinking rate were more than higher, often associated with diabetic retinopathy and diabetic vascular disease.2 Risk factorsLogistic analysis showed that the course of diabetes and HbAlc are independent risk factors for DPN(P<0.05).3 Distribution characteristics of TCM3.1 The distribution of TCM syndromeThe he results of TCM documents come from 198 DPN patients show that 117 cases(59.09%)were deficiency of vital energy syndrome, next is yin deficiency, blood stasis syndrome,phlegm-damp syndrome,heart-syndrome,Yang deficiency syndrome was 24 cases(12.12%).There were 37 cases(18.68%)with single TCM syndrome type,77 cases(38.88%)with double TCM syndrome,84 cases(42.42%)with three TCM syndromes. The results of syndrome type combination analysis showed that Qi deficiency qi and Yin deficiency syndrome,26 cases hot card 16 cases, deficiency of the 12 cases,11 cases of two virtual folder stasis syndrome of Yin and Yang,Yin deficiency syndrome, blood stasis hot phlegm,qi and Yin deficiency clip stasis syndrome,10 patients blood stasis phlegm dampness syndromes of qi deficiency and blood stasis,Yin anaemia empty folder each 8 cases of phlegm syndrome,Yin anaemia deficiency syndrome and Yin anaemia empty folder stasis syndrome 7 cases,Yang Yang deficiency of 6 cases of phlegm syndrome,Yin deficiency and blood stasis syndrome,qi deficiency and blood stasis and phlegm syndrome, blood stasis phlegm wet clip stasis syndrome, two qi stasis are in 5 cases, blood deficiency syndrome,Yang deficiency and blood stasis, deficiency of hot clip stasis each 4 cases,In addition, the fiery syndrome,phlegm-dampness syndrome,phlegm-dampness blood stasis syndrome, fire blood stasis syndrome, deficiency of qi and blood,phlegm-dampness fiery syndrome were all less than 4 times.3.2 The relationship between syndrome and disease gradeBlood stasis syndrome,phlegm-damp syndrome,Yang deficiency syndrome all increased with the disease grade,The highest frequency of mild DPN patients is qi deficiency syndrome that patients were 10 cases(5.05%),the highest frequency in moderate DPN patients is qi deficiency and Ying syndrome hat patients were 18 cases(9.09%).The highest frequency of severe DPN patients is Yin and Yang deficiency combined with turbid phlegm and blood stasis that patients were 11 cases(6.06%),and the type of syndrome in different disease groups was statistically significant(P<0.05).3.3 Correlation analysis of main TCM syndromes and clinical indicatorsThe syndrome of qi deficiency and heat were related to the value of CRE,the difference was statistically significant(P<0.05).Deficiency of Yin and Yang with stasis were related to HbAlc,the difference was statistically significant(P<0.05)Conclusion1.Compared with non-DPN,There was no difference between the two groups include sex, age, abdominal obesity rate, smoking rate, heart rate, systolic blood pressure, education level and duration of exercise, and the risk factors of DPN include disease course, body mass index, drinking history, other chronic diabetic complications, glycosylated hemoglobin,TG,liver and kidney function2.Qi-deficiency and yin-deficiency syndrome are the main TCM syndromes in DPN patients,TCM syndromes will progress from single syndromes to compound syndromes with the progress of disease, and the syndromes of severe DPN patients are mainly Yin and Yang with stasis3.Qi deficiency and heat syndrome are related to the abnormal of liver and kidney functions, the increase of Glycosylated Hemoglobin is related to Yin and Yang with stasis.
