Clinical Effect Of Gandouling Decoction Combined With Western Medicine On Psychiatric Symptoms Of Phlegm And Blood Stasis Type Wilson's Disease

Purpose:This article aims to explore the clinical characteristics of WD phlegm and blood stasis type WD psychiatric symptoms through related scales,and to observe the clinical efficacy of Gandouling Decoction combined with western medicine on psychiatric symptoms of phlegm and stasis type Wilson disease.Method:The 82 patients included were randomly divided into the experimental group and the control group,with 41 cases in each group.The control group was treated with western medicine(DMPS + Olanzapine),and the experimental group was treated with Gandouling decoction on this basis for 6 consecutive treatments.The course of treatment,before and after the treatment,the concise psychiatric scale(BPRS),unified Wilson disease rating scale(UWDRS),Hamilton depression scale(HAMD),TCM syndrome score,and 24 h urine copper,Side reaction scale(TESS)and other observations and comparisons.Result:(1)The manifestations of mental disorders of Wilson disease are diverse and cover almost all the symptoms of psychiatry.The types of mental symptoms of Wilson disease in this study can be summarized as affective disorders,mental retardation,behavioral abnormalities,personality and personality changes,etc.Types can overlap.(2)Clinical efficacy:1)Total clinical efficacy:At the end of 4 courses,the total effective rate in the observation group was 76.3%,and in the control group was 50.0%.The analysis was made,c 2 = 8.23,p = 0.041(p < 0.05),the effect of the experimental group is significantly better than the control group;the rate of the observation group at the end of 6 courses was 89.4%,in the control group was 60.6%.The analysis was made,c2=11.54,p=0.009(p < 0.01),the overall effect was significantly better than the control group.2)Comparison of TCM symptom scores:Compared with before treatment,at the end of4 courses,the observation group was p < 0.05,the control group was p > 0.05,and the comparison between groups was p < 0.05.At the end of 6 courses,the observation group was < 0.01,control group p < 0.05,comparison between groups,p < 0.01,the observation group is considered to have significant curative effect.It can be inferred that the longer the course of treatment,the greater the improvement of TCM syndrome score of WD patients with phlegm and blood stasis type WD.3)Comparison of BPRS scores:There was no significant difference in efficacy between the two groups in anxiety and depression factors and thinking disorder factors(p >0.05);the experimental group was significantly better than the control group in the lack of vitality factors,activating factors,and hostile suspicion factors(p < 0.05)It is considered that the observation group is significantly better than another in improving the BPRS score.In summary,Gandouling Decoction combined with western medicine significantly improved the symptoms of BPRS better than the western medicine group alone.4)UWDRS?score comparison: At the end of the 2 course of treatment,the comparison between the two groups was p > 0.05,and there was no significant difference in the efficacy between the two groups in improving the UWDRS? score;at the end of the 4course of treatment,p < 0.05,and the observation group was considered to have a significant effect in improving the UWDRS? score;At the end of the 6 course of treatment,p < 0.01.There have a significant effect on improving the UWDRS III score between two groups.It can be inferred that with the increase of the course of treatment,the Gandouling Decoction improves the UWDRS? score of WD patients with phlegm-stasis syndrome.5)Comparison of HAMD scores: At the end of 2 courses,there was no significant difference in efficacy between the 2 groups;at the end of 4 courses,the comparison between the two groups was p < 0.05.Compared between the two groups,p < 0.01.At the end of 6 courses,compare all to get p < 0.01.The improvement of the total score of HAMD in the observation group was significantly better than another group.6)24h urinary copper comparison: At the end of the 2 course of treatment,the comparison between the two groups was p < 0.05.It was considered that the observation group could promote the copper excretion in the body.At the end of the 4course and the 6 course of treatment,the comparison between the two groups received p < 0.01.The effect of copper removal was more significant in the pure western medicine group.In summary,Gandouling Decoction can significantly promote the elimination of copper in patients with WD disease.The longer the course of treatment,the more significant the effect.7)TESS comparison: At the end of 2 courses,comparison between the two groups,p <0.05,that the incidence of side effects of Gandouling Decoction combined with Western medicine group was significantly smaller than that of the control group,at the end of 4 courses and at the end of 6 courses,p < 0.01,it is believed that Gandouling Decoction can significantly reduce the side effects of western medicine in treating WD stagnation psychiatric symptoms.The observation group and the experimental group had no significant deterioration in safety indicators during treatment,and the safety of WD disease was considered to be higher in both groups.Conclusion:(1)The types of psychiatric symptoms of Wilson's disease are complex and the symptoms can overlap with each other;(2)phlegm and blood stasis type Wilson disease's most common emotional disorder,followed by mental retardation,abnormal behavior,and personality change;(3)Gandouling Decoction can effectively improve TCM syndrome scores of Wilson's disease with phlegm and blood stasis;(4)Gandouling decoction combined with western medicine can effectively improve the mental symptoms of phlegm and blood stasis type Wilson disease,reduce the occurrence of side reactions,and has high safety.
