Objective:1.The digital muscle assessment system was used to quantitatively detect the muscle biomechanical characteristics of patients with brain-type Wilson disease(WD)with phlegm and blood stasis syndrome of dystonia.2.To investigate the effect of hospital preparation gandouling tablet on the muscle biomechanical characteristics of brain-type WD patients with dystonia with phlegm-stasis syndrome, and to evaluate its clinical efficacy.Methods:1.60 patients with brain-type WD phlegm-stasis syndrome dystonia who met the inclusion criteria were randomly divided into treatment group(30 cases)and control group(30 cases),and their disease course,TCM classification and clinical manifestations were recorded in detail. And 30 patients were randomly selected to our hospital for physical examination as the normal group.2.The control group was given a low-copper diet and a copper drive treatment, and the copper drive drug was DMPS.Intravenous infusion of 5%GS 250ml+ DMPS(20mg/kg·d)was given once a day, followed by continuous treatment with 6d cuprate,2-d intermittent calcium supplementation for a course of treatment.Four courses of treatment.Treatment group in the control group on the basis of treatment, add gandouling tablets, three times a day, five tablets a time. The normal group did not need any medical intervention.3.Observe the biomechanical properties(F,S,D,R,C)of muscles(bilateral biceps humerus, triceps humerus, quadriceps femoris, biceps femoris, gastrocnemius(lateral),extensor longus)of WD patients and normal people.4.After 4 courses of treatment, the levels of muscle biomechanical characteristics, the scores of the modified Ashworth scale, the unified hepatolenticular degeneration scale(UWDRS),the dystonia scale(BFM),the daily living ability scale(ADL)and the TCMsyndrome scores of the patients in the treatment group and the control group before and after treatment were observed.Results:1.Compared with normal people, the muscle biomechanical characteristics of brain-type WD patients with phlegm-blood stasis syndrome of dystonia have different degrees of differences, and the measured muscle F and S levels are significantly higher than those of the normal group(P<0.01).The level of R was significantly lower than that of the normal group(P<0.01).The levels of D and C had no clinical significance compared with the normal group(P > 0.05).2.After 4 courses of treatment, the biomechanical properties of the muscles in the control group and the treatment group were improved to different degrees compared with those before treatment, and the levels of F and S of the measured muscles were significantly decreased compared with those before treatment(P<0.01).The R level was significantly higher than that before treatment(P<0.01).The levels of D and C were not statistically significant(P > 0.05).3.After 4 courses of treatment, the F,S and R levels of biceps humerus, triceps humerus, biceps femoris and quadriceps femoris in the treatment group improved more significantly than those in the control group(P<0.01).D and C comparison had no clinical significance(P > 0.05);The biomechanical properties of gastrocnemius(lateral)and extensor longus digitorum(F,S,D,R,C)were not clinically significant(P > 0.05).4.There was no statistically significant difference in the scores of the improved Ashworth scale,UWDRS score(Nerve function),BFM score,ADL score and TCM syndrome score between the control group and the treatment group before treatment(P > 0.05).After treatment, the comparison between the two groups was better than before treatment(P<0.01),and the treatment group was better than the control group, the difference was statistically significant(P<0.01).Conclusion:1.The muscle biomechanical characteristics of brain-type WD with phlegm-stasissyndrome of dystonia were significantly different from those of normal people.2.The quantitative detection of F,S and R by digital muscle assessment system can reflect the changes of muscle biomechanical characteristics in patients with brain-type WD phlegm-blood stasis syndrome of dystonia, while D and C cannot represent the muscle biomechanical characteristics in patients with this type.3.Gandou ling tablet can improve the muscle biomechanical characteristics of patients with brain-type WD syndrome of phlegm-blood stasis and dystonia.4.Gan dou ling tablet can improve the modified Ashworth scale score,UWDRS score(Nerve function),BFM score,ADL score and TCM syndrome score in patients with brain-type WD phlegm-blood stasis syndrome dystonia, with better safety and clinical efficacy.
