Objective:Understand the current status of digestive system drugs in medical institutions at all levels in Anhui Province and the common situation of drug catalogs,and analyze the reasons that affect the status quo.Taking the economic evaluation of the two types of treatment options for chronic disease ulcerative colitis as an example,the possibility of selecting medical drugs in medical institutions based on economic theory is discussed.Provide effective information support and targeted suggestions for promoting drug connection in pharmaceutical and medical institutions,and promoting the development of graded diagnosis and treatment.Methods:Part one: Survey on the trend and common status of the digestive system drugs in medical institutions at all levels in Anhui Province The purchasing data from 2016 to 2018 were obtained from 15 comprehensive medical institutions randomly selected according to hospital grade on the centralized purchasing platform of medicine in Anhui Province.The data were analyzed by R and EXCEL software in terms of drug amount,drug category,drug frequency,drug utilization and drug catalogue comparison.Part two: Research on the Selection of Listed Drugs Based on Pharmacoeconomics——Taking Infliximab as an Example The meta-analysis was used to compare the safety and effectiveness of the infliximab treatment plan and the conventional treatment plan.A Markov model of about 5 years was established to perform cost-effectiveness analysis and sensitivity analysis on the two treatment plans.Result:Part one: Survey on the trend and common status of the digestive system drugs in medical institutions at all levels in Anhui Province From 2016 to 2018,the number of purchases of digestive system drugs in primary medical institutions in Anhui Province fluctuated but the amount continued to increase.The number and amount of purchases of secondary medical institutions continued to decrease.The number and purchase of purchases of tertiary medical institutions showed a steady wave of dynamic trends.The proportion of purchasing essential drugs increases with the decrease of the level of medical institutions,while the proportion of purchasing medical insurance drugs has no obvious regularity.The results of pharmacological classification and medication synchronization performed similarly in all levels of medical institutions.The common results of the digestive system drug catalogs of medical institutions at all levels from 2016 to 2018 show that there are about 20,38 and60 kinds of digestive system medicines in the first to third-tier medical institutions.Among them,the number of the same drug varieties among medical institutions of the same level is about 7,11,and 26,and the number of the same drug varieties among different levels of medical institutions is 5,6,and 5.Part two: Research on the Selection of Listed Drugs Based on Pharmacoeconomics——Taking Infliximab as an Example Compared with the conventional treatment regimen,the infliximab treatment regimen increased ulcerative colitis by 0.06 QALY,with an increased cost of 21184.37 yuan,and an incremental cost-effectiveness ratio of 348243.88 yuan/QALY,which was higher than the willingness to pay threshold of 212676 yuan.The results of the sensitivity analysis are consistent with the basic analysis.Conclusion:The digestive system drugs equipped by medical institutions at all levels in Anhui Province have a small gap in pharmacological classification,but the large difference in specific drugs is not conducive to drug connection.The economic evaluation ofulcerative colitis shows that infliximab is not economical,and clinical needs are indeed there.Therefore,selecting drugs based on the theory of pharmacoeconomics to expand the commonality of the catalog is only the beginning.More research and summary are needed to provide better methods for drug selection..
