Objective This topic is divided into two parts,clinical research and laboratory research.The clinical research part observes the effect of Changyu Enema Recipe on the treatment of ulcerative colitis(rectal type),and evaluates the efficacy of the prescription.The experimental research part observes Changyu Enema Recipe on the feces of UC rats The effects of the middle intestinal flora and changes on serum DAO and serum D-LA values were observed comprehensively.In order to further analyze and study the possible mechanism of Changyu Enema Formula in treating ulcerative colitis.Methods 1.Clinical research: 62 cases of patients with anorectal ulcerative colitis who meet the diagnostic criteria of rectal ulcerative colitis from June 2018 to June 2019 were selected in the Department of Anorectal Diseases of the First Affiliated Hospital of Anhui University of Chinese Medicine It was randomly divided into a control group and an observation group,with 31 patients in each group.The control group was given mesalazine anal plug once every night for 2 months,and the observation group was given intestinal enema formula once every night.Twenty-two months of continuous medication,comparing the TCM syndrome scores before and after treatment in the two groups;Southerland DAI score;Baron score of rectal mucosa under the microscope;changes in serum TNF-α,IL-6,IL-17,and IL-23 levels,After the treatment,the Chinese and Western medicines were judged and the quality of life was evaluated.At the same time,the safety of medication was compared between the two groups of patients.2.Laboratory research: A total of 54 SD-grade male rats from the animal laboratory of Anhui University of Traditional Chinese Medicine were selected(Purchased from Anhui Medical University,Production license number:(Anhui Province)2017-001),weighing between 200 ± 20 g,and randomly divided into healthy group,UC saline group,UC mesalazine group,and UC low dose There are 5 groups of Changyu Enema Formula group and UC high-dose Changyu Enema Formula group,including 10 rats in the healthy group and 11 rats in each group.Except for the rats in the healthy group,the other 4 groups of rats were established UC model,24 hours after the model was established,one rat was randomly selected from each group,and the rat’s colorectal tissue was taken after anesthesia and sacrificed,and the tissue was stained with HE for pathological observation,and the model was successfully evaluated.After 72 hours,group medication was started.The rats in the UC saline group were given 0.9% sodium chloride injection 2ml enemas once a day for 15 consecutive days.The UC mesalazine group received mesalazine suppository for 1 day,In 0.9% sodium chloride injection,each rat enema 2ml daily,once a day for 15 consecutive days;UC low-dose Changyu enema formula group dissolved Changyu enema formula in 0.9% chlorine Each rat in sodium injection was enema 2ml daily,once a day for 15 consecutive days;UC was high The dose of Changyu Enema Recipe group was dissolved in 0.9% sodium chloride injection,and each rat enema 2ml once a day for 15 consecutive days(The dosage ratio is detailed in laboratory studies).After administration of the four groups of UC rats,serum DAO,serum D-LA,and intestinal flora were detected and compared with normal rats.Results 1.Clinical research results:(1)The scores of TCM syndromes after treatment in both groups were lower than before treatment.The comparison within the group was P<0.05,which was statistically significant.The observation group had a more significant decrease.The comparison between groups was P <0.05,which was statistically significant.(2)After treatment,the Southerland DAI score of both groups was lower than before treatment.The comparison within the group was P<0.05,which was statistically significant.The observation group had a more significant decrease.The comparison between the groups was P<0.05,which was statistically significant.(3)Baron scores of rectal mucosa after treatment were lower in both groups than before treatment.The comparison within the group was P<0.05,which was statistically significant,and the comparison between groups was P> 0.05,which was not statistically significant.(4)Serum levels of TNF-α,IL-6,IL-17,and IL-23 were lower in the two groups after treatment than before treatment.The comparison of various data in the group was P<0.05,which was statistically significant.For obvious reasons,the data comparison between groups was P<0.05,which was statistically significant.(5)The medication safety rate of patients in the control group was 83.87%,which was lower than that in the observation group.(6)Judgment of western medical efficacy after treatment: The total effective rate of patients in the control group was 87.10% lower than that in the observation group of 96.77%.The data comparison was P<0.05,which was statistically significant.(7)Judgment of TCM efficacy after treatment: The total effective rate of patients in the control group was 83.87% lower than that in the observation group of 93.55%.The data comparison was P<0.05,which was statistically significant.(8)Comparison of SF-36 quality of life after treatment: After treatment,the SF-36 quality of life scores of both groups were higher than before treatment.The comparison of all data in the group was P<0.05,which was statistically significant.The improvement of the 36 score was more obvious than that of the control group.The data comparison between the groups was P <0.05,which was statistically significant.2.Laboratory research results:(1)The test results of serum DAO values of the five groups of rats are in the normal group from low to high: UC high-dose observation group,UC mesalazine group,UC low-dose enema group,and UC saline group.The data of each group are different All P <0.05,statistically significant.(2)The test results of serum D-LA values of the five groups of rats are in the order from low to high in the normal group <UC high-dose observation group <UC mesalazine group <UC low-dose enema group <UC saline group,data between groups The contrast difference was P <0.05,which was statistically significant.(3)The test results of the fecal flora of the five groups of rats were better than the normal group than the UC high-dose observation group than the UC mesalazine group and the UC low-dose enema group.P <0.05,statistically significant.Conclusion 1.Rectal ulcerative colitis patients with large intestine dampness and heat syndrome have fewer adverse reactions to intestinal enemas,which can more effectively control ulcerative colitis than mesalazine.2.The mechanism of Changyu Enema Formula may be through immune regulation,reducing inflammatory factors,which can effectively protect the intestinal immune barrier,and then promote the role of promoting mucosal repair.3.This study further believed that the onset of ulcerative colitis and the disorder of the human immune mechanism caused a significant increase in various inflammatory factors.4.Intestinal microorganisms are an important part of the intestinal biological barrier,which can protect the intestinal barrier.5.The incidence of ulcerative colitis is closely related to the destruction of the biological barrier,Changyu Enema Formula can restore the intestinal biological barrier to the greatest extent,and then effectively treat ulcerative colitis.Its efficacy is better than mesalazine.
