Objective: To explore the relationship and significance of the levels of von Willebrand factor(vWF)and its lyase(ADAMTSl3)and related indexes in patients with acute myocardial infarction(AMI).Methods:1.Including 385 patients who were hospitalized in Xinjiang Uygur Autonomous Region People’s Hospital from April 1,2018 to March 1,2019 and underwent coronary angiography,which were classified into acute myocardial infarction based on their clinical manifestations and coronary angiography.2.Collect basic clinical data and initial admission values of fibrinogen,blood lipids,average platelet volume,platelet volume distribution width,platelet volume,platelet volume,platelet count,glycated hemoglobin and other indicators.In the first week of hospitalization,patients in the AMI group and the control group were collected fasting peripheral venous blood and collected blood samples.3.Double antibody sandwich solid-phase enzyme-linked immunosorbent assay(ELISA)was used to detect vWF and ADAMTS13 levels in peripheral blood of patients.4.Analyze the change of ADAMTSl3 level and its influencing factors in the two groups.Results: 1.The ADAMTSl3 levels in the normal control group and the AMI group were(1251.15)ng / ml and(744.53)ng / ml;the VWF levels were(168.15 ± 85.28)ng / ml and(233.40 ± 98.06)ng / ml.ADAMTSl3 level in the AMI group was significantly lower than the normal control group(P <0.001);while vWF level was significantly higher than the normal control group(P<0.001).The level of VWF / ADAMTS13 in the AMI group was higher than that in the normal group(2.8 vs 1.2)(P <0.001).2.Correlation analysis showed that the level of ADAMTS13 in AMI group was positively correlated with age,FIB,BNP,PLT,and PCT(P <0.05),and negatively correlated with CKMB,BMI,LVEF,and TG(P <0.05).Therewas no correlation with other vWF,troponin T,troponin I,LDL,HDL,CHO,left ventricular diastole,MPV,PWD,PLCR,HbA1c(P> 0.05).3..Logistic regression analysis results show that low levels of ADAMTS13,high levels of vWF,high age,diabetes,smoking,high levels of fibrinogen,high levels of LDL,and high levels of PDW are risk factors for acute myocardial infarction.MPV is a protective factor for acute myocardial infarction.Conclusion:The increase of vWF level and the decrease of ADAMTS13 level are closely related to the incidence of acute myocardial infarction;the lower the level of ADAMTS13 in the occurrence of acute myocardial infarction,the larger the area of myocardial infarction and the more severe the myocardial damage.Controlling patients’ body mass index and triglyceride levels may help maintain ADAMTS13 levels.
