Objective: To explore the differences of type ? and type ? collagen content and its total content,the proportion of type ? and type ? collagen and elastin content in the crura of the diaphragm and phrenoesophageal ligament tissue between the patients with giant hiatal hernia and the patients with sliding HH.Methods: According to strict inclusion and exclusion criteria,30 HH patients who underwent surgery in the department of Minimally Invasive,Hernia and Abdominal Wall of Xinjiang People's Hospital from December 2018 to December 2019 were screened,including 10 giant HH patients(case group)and 20 sliding HH patients(control group).After being reviewed and approved by the ethics committee of our hospital,the patients and their families were asked to sign the paper version of "Intraoperative Specimen Retention Informed Consent",and a total of 30 patients with the crura of the diaphragm and phrenoesophageal ligament were collected.Immunohistochemical method was used to detect the differences of type ? and type ? collagen content and its total content,proportion of type ? and type ? collagen and elastin content in the crura of the diaphragm and phrenoesophageal ligament tissue of 30 patients.Results: The content of type?collagen,total content of type?and ? collagen and their proportions,and elastin content in the crura of the diaphragm and phrenoesophageal ligament tissue of the patients with giant HH group were lower than those in sliding HH,and the content of type ? collagen was higher than those in sliding HH,they were all statistically significant(all P values were less than 0.05).Conclusions:The process of HH occurrence and development may be related to the decrease of the mechanical stability of the crura of the diaphragm and phrenoesophageal ligament tissue and the weakening of the tissue,which may be caused by the disorder of collagen metabolism and the decrease of elastin content.HH may be a local manifestation ofdisorders of collagen metabolism throughout the body.
