| Objective: To evaluate the value of Alvarado,PAS and AIR scoring systems in the conservative treatment of children with acute uncomplicated appendicitis by comparing the scoring index and risk level with the clinical index,cure rate and recurrence rate of children with acute uncomplicated appendicitis.Methods: A retrospective analysis of the case data of children with acute uncomplicated appendicitis who received conservative treatment from January 2016 to December 2018 in the People’s Hospital of Xinjiang Uygur Autonomous Region.Through conservative treatment succeed or not,it will be divided into cases conservative treatment success and failure group,cases through different scoring system to score again,it can be divided into the medium risk and high risk level,and clinical indicators,cure rates,and relapse rates were compared between different groups and risk levels.Results: 89 cases of children underwent conservative treatment.67 cases(75.28%)of conservative treatment were successful,of which 11 cases were recurrence(16.42%).Different risk levels such as PAS scoring system were different in most clinical indicators.There was no significant difference in cure rates between medium and high risk in Alvarado,PAS,and AIR scoring systems(P> 0.05).There was a significant difference in the recurrence rate of PAS scores between medium and high risk grades(P <0.05).There was no significant difference in cure rates between the scores at intermediate risk(P> 0.05).There was a significant difference in the recurrence rate between the AIR and PAS scores with moderate risk(P <0.05),and the AIR score had a higher recurrence rate with moderate risk.The high-risk recurrence rates of PAS score and AIR score were significantly different from Alvarado score(P <0.05),and the high-risk recurrence rates of PAS score and AIR score were higher than Alvarado score.In the PAS score,vomiting,T> 38.5 ℃,NC%> 85% are independent risk factors that affect the recurrence rate of different risks of conservative treatment of acute uncomplicated appendicitis in children(P <0.05).Conclusion :(1)The effectiveness of conservativetreatment of acute uncomplicated appendicitis is worthy of recognition,but there is a certain recurrence rate;(2)Alvarado,PAS,AIR scoring systems have some guidance on the results and prognosis of conservative treatment of acute uncomplicated appendicitis significance.(3)In the PAS score,vomiting,T> 38.5 ℃,NC%> 85% are independent risk factors that affect the recurrence rate of different risks of conservative treatment of acute uncomplicated appendicitis in children(P <0.05).The PAS scoring system is of high value in the conservative treatment of acute uncomplicated appendicitis in children. |
