Objective To compare the clinical effects of endoscopic retrograde cholangiopancreatography(ERCP) and surgical treatment on acute cholangitis in the elderly. Methods A retrospective case-control study was conducted to collect the clinical data of 136 elderly patients with acute cholangitis who underwent ERCP and surgery from January 2014 to January 2019 in the First Affiliated Hospital of Xinjiang Medical University.Among them,100 cases were treated with ERCP as observation group and 36 cases were treated with surgical operation as control group. The laboratory examination indexes at admission, the laboratory examination indexes on the second day after operation, operation time, intraoperative bleeding volume, hospital days after operation and the incidence of complications were compared between the two groups. Results There was no significant difference in laboratory examination indexes between the two groups at admission and the second day after operation(P > 0.05).There were significant differences in laboratory tests before and after operation(P < 0.05),except for albumin level. The operation time, intraoperative bleeding volume and hospital stay in the observation group were significantly lower than those in the control group(P < 0.05);the incidence of complications in the observation group was 5.00%(5/100),lower than that in the control group(16.67%(6/36)(P < 0.05).Conclusion ERCP has a significant effect on elderly patients with acute cholangitis. It can effectively reduce trauma, shorten hospitalization days and reduce the incidence of adverse reactions.ERCP should be actively applied in clinical work.
