Objective: To investigate the features of morphological characteristics,hemodynamic parameters and apparent diffusion coefficient(ADC)value in invasive lobular carcinoma(ILC)on MRI,and the relationship between the MRI features and Ki-67 index.Methods:184 patients with ILC and invasive breast carcinoma(IDC)confirmed pathologically who underwent breast MRI from January 2014 to December 2018 were analyzed retrospectively,including 52 patients with ILC and 132 patients with IDC,aged26-68 years,with an average age of about 46.9 years.Preoperative MRI examination was performed,and all patients did not receive invasive examination and treatment before the MRI examination.Imaging findings were retrospectively evaluated according to the 2013 MRI BI-RADS lexicon.Results:Compared to IDC,patients with ILC tended to have a higher incidence of multifocality and lower incidence of breast conserving surgery histopathologically(P < 0.05).In terms of immunohistochemistry,the positive rate of steroid hormone was higher and the expression level of ki-67 index in ILC was lower than those in IDC(P < 0.001).Non-mass-like enhancement on MRI was more frequently seen in the ILC group,and mass-like enhancement was more frequently seen in the IDC group(P < 0.001).ADC and TTP values in the ILC group were higher than those in the IDC group(all P < 0.05),and there were no significant statistical difference in early enhancement ratio,maximum enhancement ratio,WI,WO and PEI values(all P >0.05).The mass-like enhancement and early enhancement ratio were positively correlated with Ki-67 index(both P < 0.05).And Ki-67 index of ILC group was negatively correlated with ADC value(P < 0.001).Conclusion:MRI is of high diagnostic value for ILC.ILC is more likely to present with non-mass-like enhancement on MRI,and peak eanhancemnet is reached relatively late.Ki-67 index of ILC was positively correlated with mass-like enhancement and early enhancement rate,and negatively correlated with ADC value.
