Objective:To study the relationship between obesity and brain metastasis of breast cancer and its prognostic.Methods:Retrospective analysis of detailed case data and follow-up data from January 2010 to January 2018 of the tumor hospital aff-iliated to Xinjiang Medical University.The primary lesion was diagnosed as breast cancer by surgical pathology,and regular systemic treatment(including surgery,chemotherapy,Endocrine therapy and targeted therapy),348 patients with breast metastases diagnosed by CT and/or MRI,and all patients received radiotherapy.Patients were divided into two groups according to the Chinese Body Mass Index(BMI):Non-obese(BMI<28 kg/m2)and overweight(BMI ? 28 kg/m2).Baseline of the two groups was compared,including age at diagnosis of brain metastasis,menopausal status,KPS score,pathological type,histological grade,mass size at first diagnosis,lymph node positive number,molecular typing,Ki-67,intravascular tumor thrombus,visceral metastasis,number of metastatic organ sites.The Cox proportional hazard model was used to detect BMI and other factors related to survival,and the survival curve was generated using GraphPad Prism 8.0 software for survival rate(OS)analysis.Resules:BMI? 28 kg/m2(HR=1.659,P<0.001)was associated with increased mortality.And the age at which breast cancer was diagnosed with brain metastasis(HR=1.602,p=0.026),KPS score(HR=2.733,p=0.002),number of metastatic organs(HR=1.515,p=0.004),triple-negative breast cancer(HR=1.327,p=0.053)were poor prognostic factors in patients with brain metastases from breast cancer.Moreover,compared with BMI<28 kg/m2(n=180),the median survival time of BMI?28 kg/m2(n=168)was significantly shortened(11 months vs 14 months;p=0.001).Conclusion:Obesity may have a negative impact on the survival rate of patients with brain metastases from breast cancer.As a risk factor for patients with brain metastases from breast cancer,it should be used as a stratified prognostic indicator for this type of patient population in future clinical trial design.
