Risk Factors And Clinical Characteristics Of Coronary Heart Disease In Early And Late Menopausal Women

Objective:To explore the factors influencing the incidence of coronary heart disease in postmenopausal women at different periods.Methods:844 postmenopausal female patients who received coronary angiography were selected,among whom 561 patients had coronary artery stenosis of different degrees and all of them met the diagnostic criteria of coronary heart disease,as the observation group(coronary heart disease group).The results of coronary angiography in 283 patients were not abnormal and were used as the control group.According to nationality and age of menopause,it was further divided into:the han menopausal ?6 years coronary heart disease group(n=70),the uygur menopausal?6 years coronary heart disease group(n=58),the han menopausal ?10 years coronary heart disease group(n=327),the uygur menopausal ?10 years coronary heart disease group(n=106),the han menopause ?6 years control group(n=76),the uygur menopause?6 years control group(n=40),the han menopause ?10 years control group(n=129),and the uygur menopause ?10 years control group(n=38).The differences of clinical characteristics,biochemical indexes and coronary artery disease characteristics between each group were compared and analyzed.Results:Multi-factor analysis showed that hypertension,diabetes,and low apolipoprotein AI were independent risk factors for coronary heart disease in han nationality menopausal ?6 years women.Hypertension and high LDL-C were independent risk factors for coronary heart disease in Uyghur nationality menopause ?6 years women.Elderly age,hypertension,carotid atherosclerosis and high LDL-C were independent risk factors for coronary heart disease in han nationality menopausal ?10 years women.Hypertension,high LDL-C and low HDL-C were independent risk factors for coronary heart disease in Uyghur nationality menopausal?10 years women.The majority of coronary artery lesions were single vessel lesions in the han nationality and uygur post-menopausal ?6 years group,while the proportion of coronary artery lesions with double or multiple vessels in the post-menopausal ?10 yearsCHD group was higher than that in the post-menopausal ?6 years CHD group.The rates of three lesions in the uyghur menopause ?6 years coronary heart disease group and the uyghur menopause ?10 years coronary heart disease group were higher than those in the han menopause ?6 years coronary heart disease group and the han menopause ?10 years coronary heart disease group,respectively.Conclusions:Hypertension,diabetes and low apolipoprotein AI were independent risk factors for CHD in early menopausal women of han nationality,while advanced age,hypertension,carotid atherosclerosis and high LDL-C were independent risk factors for CHD in late menopausal women of han nationality.Hypertension and high LDL-C were independent risk factors for coronary heart disease in early menopausal women of uygur nationality,while hypertension,high LDL-C and low HDL-C were independent risk factors for coronary heart disease in late menopausal women of uygur nationality.There are some differences in risk factors of coronary heart disease between han and uygur postmenopausal women.Coronary artery lesions in early menopausal female patients with coronary heart disease were mainly single-vessel lesions.With the extension of menopause,the range of coronary artery lesions gradually increased.Uyghur postmenopausal women with coronary heart disease have a high rate of three-vessel disease,and the severity of coronary artery disease is more serious than that of han women.
