ObjectiveAnalyze the clinical effects of hemostatic drugs injected or sprayed by gastroscopy on actue nonvariceal upper gastrointestinal bleeding.The total effective rates,adverse reactions among the three groups are compared,and instant hemostasis rate,recurrent bleeding rates,and emergency operation rates between the gastroscopy spraying group and the gastroscopy injection group are compared to evaluate the effect of gastroscopy spraying and injection of hemostatic drugs.Methods105 patients of upper gastrointestinal bleeding are randomly divided into control group including 30 cases,the gastroscopy spraying group(spraying group for short)includ ing 33 cases,and the gastroscopy injection group(injection group for short)including 42 cases.The control group is given intravenous PPIs,without the use of special hemostatic drugs.On the base of control group,the spraying group is sprayed reptilase additionally,while the injection group is injected 1:10000 epinephrine additionally.In the end,clinical effects,adverse reactions among three groups are recorded to evaluate effect of gastroscopy hemostasis drugs.The instant hemostasis rates,recurrent bleeding rates,emergent operation rates of the spraying group and injection group are compared.Results1.The difference of the total effective rates among the three groups is statistically significant.The total effective rates of the spraying group and injection group are higher than that of the control group,and it is higher in the injection group than that of the spraying group,and differences are statistically significant.2.Instant hemostasis rate is significantly higher in the injection group than that of the spraying group.3.Recurrent bleeding rate of the injection group is lower than that of the spraying group.The difference is statistically significant.4.There is no difference in emergent operation rate between the spraying group and injection group.5.There is no significant difference in adverse reactions among the three groups.Conclusions1.The injection of hemostatic drugs guided by gastroscopy is a rapid and effective treatment on actue non-variceal upper gastrointestinal bleeding,and its effect is more remarkable than intravenous PPIs.2.The effect of local injection of 1:10000 epinephrine by gastroscopy is better than that of spraying reptilase.
