ObjectiveThrough determination the plasminogen and D-dimer in hematoma and plasma of the hypertensive intracerebral hemorrhage patients,to explore the correlation of intracranial hematoma fibrinolytic activity and fibrinolytic activity in plasm of hypertensive intracer ebral hemorrhagic patients.Research MethodsChoose 20 patients with hypertensive intracerebral hemorrhagic who are in Shandong traffic hospital and received intracranial hematoma micro invasive craniopuncture during January 2014 to December 2015.At the same time,a total of 20 patients were selected as health control group who did health examination.Extraction from the surgery group patients hematoma samples、plasma samples and control group plasma samples,3000 rpm speed centrifugation for 10 minutes,the supernatants were harvested to cryogenic vials and marked,placed at the Ultra-low temperature freezer stay at-80℃prepared test.The supernatant fluid plasminogen and D-dimer levels were measured with an enzyme-linked immunosorbent assay(ELISA).Data was analyzed with SPSS 13.0 statistic software,it was considered statistically significant,when P≤0.05.Result:1.The level of plasminogen in hematoma samples were significantly higher than those in plasma samples of the surgery group patients(P<0.05),the plasma samples of the surgery group patients average plasminogen was 12.36±3.54 ng/ml,the hematoma samples of the surgery group patients average plasminogen was 14.99±3.70ng/ml.The level of D-dimer in hematoma samples were more than those in plasma samples of the surgery group patients(P > 0.05),the plasma samples of the surgery group patients average D-dimer was 737.61±166.34ng/ml,the hematoma samples of the surgery group patients average D-dimer was 788.15±140.62ng/ml.2.The level of plasminogen in plasma samples of the surgery group patients were more than those in plasma samples of the control group patients(P > 0.05),the plasma samples of the surgery group patients average plasminogen was 12.36±3.54 ng/ml,the plasma samples of the control group patients average plasminogen was 10.72±2.98 ng/ml.The level of D-dimer in plasma samples of the surgery group patients were more than those in plasma samples of the control group patients(P>0.05),the plasma samples of the surgery group patients average D-dimer was 737.61 ±166.34ng/ml,the plasma samples of the control group patients average D-dimer was 639.75±258.18ng/ml.The level of plasminogen in hematoma samples of the surgery group patients were more than those in plasma samples of the control group patients(P > 0.05),the hematoma samples of the surgery group patients average plasminogen was 14.99±3.70ng/ml,the plasma samples of the control group patients average plasminogen was 10.72±2.98ng/ml.The level of D-dimer in hematoma samples of the surgery group patients were more than those in plasma samples of the control group patients(P < 0.05),the hematoma samples of the surgery group patients average D-dimer was 788.15±140.62ng/ml,the plasma samples of the control group patients average D-dimer was 639.75±258.18 ng/ml.3.Correlation between hematoma fibrinolytic activity and the plasma fibrinolytic activity need the Pearson correlation test,plasminogen in hematoma with plasminogen in plasma R=0.627,P=0.00<0.05.It indicated that the hematoma and the plasma fibrinolytic activity had significant correlation.D-dimer in hematoma with D-dimer in plasma R=-0.002,P=0.992>0.05.There is no sufficient reason to think there is a correlation between the two.4.Surgical patients were divided into two subgroups which suction the fresh blood in operation group and fresh blood was not found in operation group.The level of plasminogen in suction the fresh blood in operation group were significantly higher than those in fresh blood was not found in operation group(P<0.05),the average plasminogen of the hematoma samples in suction the fresh blood in operation group was 17.62±3.37ng/ml,the average plasminogen of the hematoma samples in fresh blood was not found in operation group was 12.90±3.28ng/ml.The average plasminogen of the plasma samples in suction the fresh blood in operation group was 11.77±3.32ng/ml,the average plasminogen of the plasma samples in fresh blood was not found in operation group was12.61±3.71ng/ml,the difference was not statistically significant(P>0.05).5.Correlation between hematoma fibrinolytic activity and the onset time,need theSpearman rank correlation test,plasminogen in hematoma with the extended course of disease,R=-0.120,P=0.624,there is no sufficient reason to think there is a correlation between the two;D-dimer in hematoma body with the extended course of disease,R=0.285,P=0.237,there is no sufficient reason to think there is a correlation between the two.ConclusionThe hematoma in hypertensive intracerebral hemorrhage patients was exist plasminogen,and the average plasminogen in hematoma were higher than those in the plasma(P<0.05),and there was a significant correlation between them;which not only provides a theoretical basis for the use of urokinase in minimally invasive surgery for cerebral hemorrhage,it can also be used to measure the content of plasminogen in the plasma to estimate the content of plasminogen in the hematoma,it has important significance for the use of urokinase in the operation.The level of plasminogen in suction the fresh blood in operation group were significantly higher than those in fresh blood was not found in operation group,two groups have significant difference(P<0.05).Whether this is an independent risk factor for cerebral Re-hemorrhage,need further research and clinical data support.
