BackgroundAlthough the incidence and mortality of colorectal cancer are different among different countries,it is undeniable that colorectal cancer is one of the common malignancies in the world. The incidence is high in Europe and the United States, and the morbidity of male and female and mortality were the 3rd.In recent years, due to the improvement of living standards, the incidence of colorectal cancer may have an increasing trend in China;the prognosis related factors of colorectal cancer patients include TNM stage, carcinoembryonic antigen level,KPS score, and tumor cell differentiation level. At present, serum sodium levels may be associated with poor prognosis in patients with colorectal cancer. If early detection, monitoring, and management are available, and individualized treatment plans can be developed,it may improve patient outcomes and improve prognosis.ObjectiveTo investigate the clinical manifestations and diagnosis basis of patients with colorectal cancer with hyponatremia, which is conducive to the early diagnosis and timely and effective treatment of the disease;then the prognostic analysis of such patients to explore whether hyponatremia is the prognostic factor of colorectal cancer patients.MethodsA retrospective analysis was performed on 113 patients with pathologically confirmed colorectal cancer who were admitted to the Affiliated Hospital of Taishan Medical College from February 2012 to February 2015.The patients were classified as having hyponatremia based on their serum sodium levels after admission for the first time. The group and the normal group of sodium were compared to compare the clinical manifestations and serological examinations between the two groups. The survival period of patients was followed by telephone.SPSS 20.0 was used for statistical analysis on data analysis. The correlation analysis between serum sodium levels and clinical factors was analyzed by Chi-square test.Logistic regression analysis was used for single-factor and multi-factor survival analysis.P < 0.05 difference was statistically significant.ResultIn 113 patients with colorectal cancer,29 patients were associated with hyponatremia, accounting for about 25.7%.In patients with hyponatremia, except for one case of severe hyponatremia, the clinical manifestations of neurological symptoms such as confusion and confusion, mild to moderate patients have no obvious symptoms,or only weak, nausea, vomiting and other non-specific performance Among them,16 cases were asymptomatic,7 cases were nausea and vomiting, and 5 cases were asymptomatic; In the correlation analysis between the levels and clinical factors, there was no statistically significant difference in serum sodium levels between the two groups in terms of gender, tumor cell differentiation, and tumor site(P>0.05),age,tumor TNM stage, white blood cell count, and carcinogen The difference in antigen and hemoglobin levels was statistically significant(P<0.05).In the univariate logistic regression analysis, there was no significant difference in the three-year survival outcome between the two groups in terms of age, gender, and tumor site(P>0.05).There were statistically significant differences in carcinoembryonic antigen level, serum sodium level,TNM stage, and tumor cell differentiation(P<0.05),and the odds ratio(OR)was>1.In multivariate logistic reg-ression analysis, two groups of patients The 3-year survival outcome was still statistically different in terms of carcinoembryonic antigen levels, serum sodium levels,TNM stages, and tumor cell differentiation, and the odds ratio(OR)was>1.ConclusionIn patients with colorectal cancer and hyponatremia, except for neurologic symptoms in patients with severe hyponatremia, generally no specific clinical manifestations;serum sodium levels in patients with colorectal cancer and its sex, tumor cell differentiation, tumor site No correlation, but with age, tumor TNM stage, white blood cell count, carcinoembryonic antigen and hemoglobin levels;Logistic regression analysis showed that: tumor TNM stage, tumor cell differentiation, carcinoembryonic antigen and serum sodium levels are affected the risk factors for colorectal cancer patients' 3-year survival rate were higher levels of cancer antigen, poorer differentiation, later tumor stages, and lower serum sodium levels, all indicating lower rate in patients with 3-year survival.
