Objective:To analysis the effects of clear aligner and fixed appliance in malocclusion patients with mild to moderate chronic periodontitis.This experiment measurement some parameters before and after orthodontic treatment-the root length,the alveolar bone height and the alveolar bone area around the root of the upper and lower anterior teeth.To explore the clinical effects of orthodontics in different appliances,and to provide more comprehensive basis and guidance for risk assessment and clinical design of orthodontic tratment in adult periodontitis patients.Methods:Selected 20 patients during 2016 to 2019 from the Department of Orthodontics,Affiliated Stomatological Hospital of Kunming Medical University.Including 10 males and 10 females,ages from 20 to 40.Grouped according to different appliances,divided into invisible group(group A)and fixed group(group B).Using Dolphin Imaging software to ananysis patients CBCT data before(T1)and after(T2)orthodontic treatment.Results:?The average age of the invisalign group was 29.86±4.01 years old,and the mean age of the fixed group was 30.16±4.11 years.There was no significant difference between the two groups(P>0.05).The average treatment of the invisalign group was 23.60±3.81 months.The average treatment of the fixed group was 25.60±4.97 months,there was no significant difference between the two groups(P>0.05):?The length of the invisalign group was reduced by 0.14±0.14 mm,and the fixed group was reduced by 0.72±0.35 mm in patients with mild to moderate chronic periodontitis.The difference was statistically significant,and the fixed group was significantly higher than the invisible group(P<0.01);?The labial height of the invisalign group was reduced by 0.4910.49mm,the lingual side was reduced by 0.63±0.24mm,the distal side was reduced by 0.08±0.19mm,the mean height of the four different sites was decreased by 0.12±0.16mm.The difference was statistically significant(P<0.01).The proximal side increased by 0.07±0.22mm,and the difference was statistically significant(P<0.05).The fixed group was reduced by 0.27±0.53mm,the lingual side increased by 0.21±0.45mm,the proximal side increased by 0.34±0.44mm,and the mean height of four different sites increased by 0.18±0.23mm The difference was statistically significant(P<0.01);The distal middle was reduced by 0.02±0.22 mm,and there was no statistically significant difference(P>0.05).The invisible group height measurement was lower than the fixed group except of the near-far side height measurement(P<0.05).The area of the alveolar bone in the invisalign group increased by 0.42±2.82 mm2,the lingual alveolar bone decreased by 0.32±4.38 mm2,and the total area increased by 0.19±4.24 mm2.There was no significant difference after orthodontic treatment(P>0.05);The alveolar bone area of the fixed group decreased by 0.78±3.54 mm2,the lingual decreased by 2.54±6.6 mm2,and the total area decreased by 3.96±6.46 mm2.The difference in area reduction after orthodontic treatment was statistically significant(P<0.01).Conclusions:1.The root length was reduced,and the resorption of the fixed appliance group was more serious than the invisalign group after orthodontic treatment;2.In adult patients with mild and moderate chronic periodontitis,changes in alveolar bone height at different sites were different after orthodontic treatment,and the increase in alveolar bone height on the labial and lingual side of the invisible group was better than that of the fixed group;3.There was no change in the total area and the total area of labial and lingual of alveolar bone treated with invisible orthodontic appliance;After orthodontic treatment with fixed orthodontic appliance,the area of the labial side and the lingual side decreased.The alveolar bone area decreased more in fixed group than in invisible group;4.For adult orthodontic treatment with mild and moderate chronic periodontitis,invisible correction has relative advantages.
