Objective:to compare the clinical effect of ondansetron combined nalmefene or ondansetron or nalmefene on prevention of postoperative nausea and vomiting in patients undergoing gynecologic laparoscopic surgeries.Methods:A total of 200 patients,aged 20 to 50 years,American-Society of Anesthesiologists(ASA)physical status ? or ?,scheduled for gynecologic laparoscopic surgery under general anesthesia were randomly divided into four groups(n=50):placebo-group(A),ondansetron-group(O),nalmefene-group(N),ondansetron combined nalmefene(ON).All patients received general anesthesia.Group A received 4 ml of normal saline before the end of the operation;Group O received 8 mg of ondansetron before the end of the operation,Group N received 0.25?g/kg of nalmefene after the end of the operation,Group ON received 8 mg of ondansetron and 0.25?g/kg of nalmefene after the end of the operation.All patients were administered propofol combined with remifentanil to maintain anesthesia.No patient-controlled analgesia was used in all patients.Each patient was observed and asked for the degree and frequency of PONV within 24h after the operation.Results:The incidences of PONV(???)in Group A,Group O,Group N,and Group ON were 68%,36%,32%and 16%respectively.The incidences of PONV in group O?group N and group ON were significantly lower than those in group A.There was a significant difference between Group O and Group A in incidence of PONV.The difference of PONV was significant between Group N and Group A.There was a significant difference between Group ON and Group A in incidence of PONV.The incidence of PONV in group ON was significantly lower than that in group O and group N.The difference between the group ON and group O was statistically significant.The difference between the group ON and group N was significantly different.Conclusion:Ondansetron or nalmefene is effective in preventing postoperative nausea and vomiting in patients undergoing gynecologic laparoscopic procedures;ondansetron combined with nalmefene is better than ondansetron or nalmefene alone in preventing postoperative nausea and vomiting in patients undergoing gynecologic laparoscopic surgeries. |
