Objectives:1.To construct a comprehensive management system for clinical teachers of nursing undergraduates with clear orientation and in line with the current situation in the province,so that the selection of clinical teachers can be based on evidence;2.It is convenient for clinical teachers to clarify their own development needs,and provide a theoretical basis for measuring the comprehensive ability of clinical teachers;3.Promote continuous and steady improvement of the overall quality of clinical teachers through regular training and evaluation of clinical teachers of nursing undergraduates;4.To standardize and rationalize the management process for clinical teachers of nursing undergraduates,effectively improve the teaching level and ability of clinical teachers,ensure high-quality nursing practice education,and improve the comprehensive quality of nursing undergraduates.Methods:1.Adopting the purpose sampling method and following the principle of information saturation,a total of 36 nursing managers,clinical teachers of nursing undergraduates,and nursing undergraduate interns were selected for semi-structured interviews from the third-grade hospitals and medical universities of Yunnan Province in October 2017 to April 2018.1n order to refine topical informations related to the five dimensions of selection conditions,qualifications,comprehensive competence,teacher training,and evaluation methods.2.Based on the results of semi-structured interviews,the expert consultation questionnaire was compiled by combining the contents of the literature review.Using the purpose sampling method,28 consultant experts who came from 2 universities and 10 third-grade hospitals in Kunming,Yuxi,Qujing,Gejiu of Yunnan Province were selected for two-round Delphi expert consultations in April to August 2018.The mean value of important score,coefficient of variation and weight of each index were calculated.The reliability and representativeness of consultant experts were analyzed by positive coefficient,authority coefficient and coordination coefficient.The final draft of the system was preliminarily formed.3.Based on the final draft of the system,questionnaires of student assessment and clinical teacher self-assessment was designed.Using the purpose sampling method,200 pairs of clinical nursing teachers and their undergraduate interns were selected to participate in the investigation from five third-grade hospitals of Yunnan Province in September to November 2018.In order to test the reliability and validity of questionnaires and establish the final system.Results:This study was divided into three stages:1.The first stage:This stage interviewed 8 nursing managers,10 clinical teachers of nursing undergraduates,18 nursing undergraduate interns,and transcribed more than 140,000 words.19 second-level indicators and 54 third-level indicators related to 5 first-level indicators,namely,selection conditions,qualification,comprehensive ability,teacher training and evaluation methods were extracted.2.The second stage:Through two-round Delphi expert consultations,the final draft of the comprehensive management system consisting of 5 first-level indicators,21 second-level indicators and 62 third-level indicators was preliminarily formed.The mean value of the importance score of the first-level indicators from high to low was comprehensive ability(4.92),teacher training(4.79),selection conditions and qualification(4.75),evaluation methods(4.67).The mean value of the second-level indicators ranged from 3.87 to 4.92,and the mean value of the third-level indicators ranged from 3.96 to 5.00.In this stage,The original number of consulting experts was 28.The age of the experts ranged from 37 to 56(48.68±5.48),and the working year ranged from 11 to 38(29.57±6.51).The positive coefficients of the two-round expert consultations were 96.43%and 88.89%,the authoritative coefficients were 0.885 and 0.890,and the coordination coefficients of indicators at all levels ranged from 0.224 to 0.355 after two-round consultations.The results showed that the consultant experts had good reliability and representativeness(P<0.001).3.The third stage:200 pairs of questionnaires were issued in this study.187 pairs of questionnaires were collected,the questionnaire recovery rate was 93.50%,and then 157 pairs of valid questionnaires were collected,the effective questionnaire recovery rate was 83.96%.According to statistical analysis,the reliability Cronbach's a coefficient was 0.946,the Cronbach's ? coefficient of each dimension ranged from 0.756 to 0.978.The content validity I-CVI ranged from 0.83 to 1.00,the S-CVI was 0.995.The structural validity extracted 19 common factors by exploratory factor analysis,the cumulative contribution rate was 77.03%,and the variance contribution rate of each item was more than 57.1%.The absolute load value of all items on the corresponding common factor was more than 0.4,ranging from 0.509 to 0.875.The structure of questionnaires was simple and clear.The above data showed that the reliability and validity of questionnaires were good.Conclusions:This study constructed a comprehensive management system for clinical teachers of nursing undergraduates in Yunnan Province through semi-structured interviews,literature review,Delphi expert consultation and reliability and validity test.The system can provide a theoretical basis for each teaching hospital in the province to formulate the admission and management process of clinical teachers adapted to the hospital.And also provide references for the selection,certification and training for clinical teachers of nursing undergraduates.
