Objective:This study aims to explore the relationship between tinnitus frequen cy matching,tinnitus loudness matching rule and hearing loss frequency and he aring loss degree in tinnitus patients,and to provide reference basis for tinnitus treatment by comparing tinnitus severity before and after acoustic therapy.Methods:A total of 100 patients with primary tinnitus who were admitted to t he second affiliated hospital of kunming medical university from February 2018 to December 2018 were selected as the chief complaint.Basic information of the patients was asked,including age,gender,tinnitus side,course of disease and accompanying symptoms.Professional otology examination was performed,p erfect the related audiology examination(pure tone threshold audiometry,etc.),tinnitus careful examination(including frequency of tinnitus,tinnitus loudness match,Tinnitus masking match),severity and told the patient to complete the t innitus questionnaire to assess tinnitus severity,and tinnitus treatment,using thr ee different features sound(standard stimulation,TRT sound and background mu sic),according to the results of the tinnitus patients with frequency matching c hoose effective frequency,and as a "prescription" by the transducer output,acti ng on the patients with auditory pathway,To the extent that you can’t hear th e tinnitus,once a day,20~30 minutes per time,5 days was a course of treat ment,and patients were instructed to receive acoustic therapy at the same time for 5 consecutive days.After the end of 5 times of acoustic therapy,patients were asked to complete the tinnitus severity scale again,to assessed tinnitus sev enty again,Results:①Among the 100 patients with tinnitus,49 were males and 51 were females.The prevalence of tinnitus was basically the same between males and females(P=0.552,>0.05),and there was no significant difference in the incid ence between males and females.The mean age of males(50.80±13.61 years)and females(49.04±15.89 years).The male and female ages were tested by L evene test of homogeneity of variance F=1.248,P=0.27(>0.05);②Tinnitus fr equency matching results:there were 16 cases with low frequency(≤1000Hz),32 cases with intermediate frequency(1000~4000Hz),and 52 cases with high frequency(≥4000Hz).Similarly,hearing loss was found in 14 cases with low frequency(≤1000Hz),33 cases with intermediate frequency(1000-4000Hz),a nd 53 cases with high frequency(≥4000Hz).It can be seen that tinnitus freque ncy matching is dominated by high frequency,and hearing loss frequency is al so the high frequent.According to Pearson correlation analysis,there is a corre lation between them(correlation coefficient r=0.847,(P<0.05);③Tinnitus to udness matching results:≤25dB:29 cases,26~40dB:27 cases,41~55dB:12 cases,55~70dB:18 cases,71~90dB:14 cases,>90dB:0 cases.Results of hearing loss(subject to the maximum degree of hearing loss):≤25dB:31 cases,26~40dB:26 cases,41~55dB:14 cases,56~70dB:17 cases,71~90dB:12 cases,>90dB:0 c ases.Tinnitus loudness matching and hearing loss were mostly concentrated in≤25dB.Through Pearson correlation analysis,they were correlated(correlation coefficient r=0.811,P<0.01);④Rate of sound therapy at different frequenc ies:≤1000Hz:93.75%,1000~2000Hz:78.57%.2000~4000Hz:83.33%;4000~600 OHz:73.91%;6000~8000Hz:75.86%.Kruskal-wallis test of multiple independent samples showed that the effect of acoustic therapy on tinnitus with different fr equencies was different(P=0.004,<0.05),and the effect on patients with low-frequency tinnitus was better than that on patients with high-frequency tinnitus.However,spearman correlation coefficient was 0.304,indicating that the correla tion between acoustic therapy and tinnitus frequency was not very large;⑤Ac oustic therapy for different tinnitus loudness effect:≤25dB:75.86%,26-40dB:81.48%,41~55dB:100%,56~70dB:77.78%,71~90dB:71.42%.Spearman correlatio n coefficient bilateral test P=0.864(>0.05)indicated that the correlation coeffi cient was not statistically significant indicating that tinnitus loudness had nothi ng to do with therapeutic effect;⑥Therapeutic effects of different degrees of hearing loss:≤25dB:70.97%,26~40dB:88.46%,41~55dB:78.57%,56~70dB:82.35%,71~90dB:83.33%,>90dB:0 cases;The bilateral test of Spearman correlatio n coefficient P=0.445(>0.05)indicated that the correlation coefficient was not statistically significant,that is,the degree of hearing loss was unrelated to the treatment effect;⑦Comparison of tinnitus severity before and after acoustic t herapy:according to the evaluation table of tinnitus severity before treatment:]evel 0:0 people,level 1:12 people,level 2:24 people,level 3:30 people,level 4:21 people,level 5:10 people,level 6:3 people.After 5 acoustic therapy,the effi cacy was evaluated again through the tinnitus severity assessment table:level 0:7 people,level 1:36 people,level 2:27 people,level 3:18 people,level 4:9 peo pie,level 5:1 people,6:2 people.Paired t-test showed that the curative effect of the patients before and after treatment had significant statistical significance(c orrelation coefficient 0.847,P<0.01).Among the patients receiving acoustic t herapy,7 patients were cured,and the total effective rate of tinnitus reached 80%.Conclusions:1.The prevalence of tinnitus was almost equal between men and women(P=0.552,>0.05),and there was no significant difference between m en and women in the incidence of tinnitus.The average age of males was 50.80±13.61 years old,and the average age of females was 49.04±15.89 years old 2.Tinnitus frequency matching is significantly correlated with hearing loss freq uency;3.Tinnitus loudness matching is significantly correlated with hearing loss;4.Tinnitus severity is significantly correlated with tinnitus loudness;5.Acoustic therapy has a good therapeutic effect on tinnitus.The therapeutic effect on tin nitus of different frequencies is different,but the correlation is not significant.6.There was no statistically significant effect of acoustic therapy on different de grees of hearing loss;7.The effect of acoustic therapy on tinnitus with differen t loudness was not statistically significant.
