Objective:To observe the clinical efficacy of Ortho-Bridge System, locking compression plate and intramedullary nail system in the treatment of long bone fractures of lower extremities,And statistical analysis.By comparison of the statistical indexes of the three groups,To explore the advantages and disadvantages of three fixed methods for the treatment of long bone fractures of lower extremities, and provide theoretical basis and reference for clinicians to treat long bone fractures of lower limbs. Methods:A retrospective analysis method was used to collect patients with long bone fractures of the lower extremities from June 2017 to June 2018 in the Department of Traumatology,Second Affiliated Hospital of Kunming Medical University.According to the inclusion criteria and exclusion criteria, the subjects were selected.A total of 64 patients, including 38 males and 26 females, aged 18-64 years, were classified according to long bones(AO/OTA),including 19 cases of A1 type,16 cases of A2 type,9 cases of A3 type, and 6 cases of B1 type.1 case of B2 type,3 cases of B3 type, and 4 cases of C1 type,0 case of C2 type, and 6 cases of C3 type.According to the internal fixation method, there were 19 cases of Ortho-Bridge System(OBS)group,21 cases of locking compression plate group and 24 cases of intramedullary nail system group. The patients were followed up for 6-18 months, with an average of 11.93±3.25 months. The operation time,Surgical bleeding volume and fracture reduction of the three groups were recorded. The follow-up patients were reviewed for X-rays to record the fracture healing time, and the postoperative complications and surgical outcomes were analyzed and compared. The treatment effect was evaluated according to the Johner-Wruhs functional scoring standard in 9 months later. The advantages and disadvantages of the three treatment methods were evaluated by statistically processing the statistical data of the three groups of cases. Results:All patients received good follow-up.The age, gender, and fracture side of the three groups were statistically analyzed,P>0.05,the difference was not statistically significant, and the three groups were comparable.Intramedullary nail group operation time was 136.25±31.73min,surgical bleeding volume was 110.00±32.83ml,fracture anatomical reduction was performed in 6 cases, functional reduction was in 18 cases, fracture healing time was 15.26±2.63 weeks, and one case occurred nonunion after operation. The limb function excellent rate of evaluation was 87.5%.The operation time of LCP group was 148.09±30.76min,surgical bleeding volume was 94.28±18.59ml,18 cases of fracture anatomical reduction,3 cases of functional reduction, fracture healing time 14.52±2.66 weeks,1 case of postoperative infection,The limb function excellent rate of evaluation was 90.5%.The operation time of OBS group was 114.47±20.74min,the surgical bleeding volume was 94.21±20.90ml,the fracture anatomical reduction was 18 cases, the functional reduction was 1 case, the fracture healing time was 14.87±2.82 weeks,no adverse complications occurred after operation, and the excellent rate of limb function evaluation was 89.5%.The operation time of OBS group was significantly different from that of intramedullary nail group and LCP group,P<0.05.The overall blood loss of the three groups was P>0.05,the difference was not statistically significant, but the comparison between groups showed there was a statistically significant difference between the intramedullary nail group with the OBS group and the LCP group. The difference in the amount of surgical bleeding between the OBS group and the LCP group the P>0.05,The difference was not statistically significant.About fracture reduction,There was significant difference between the intramedullary nail group with the OBS group and the LCP group. The difference was statistically significant. But difference between the OBS group and the LCP group was P>0.05.The difference was not statistically significant. The fracture healing time comparison of the three groups was P>0.05,The difference was not statistically significant. The comparison of complications between the three groups was P>0.05,the difference was not statistically significant;the three groups of functional evaluation was P>0.05,no statistical difference, the treatment effect was equivalent. Conclusions:Compared with the intramedullary nailing group and the LCP group, the OBS group had similar therapeutic effects.OBS,LCP,and intramedullary nail are good internal fixation methods for the treatment of long bone fractures of the lower extremities. However, the OBS group had shorter operative time, less bleeding and higher anatomical reduction rate of fractures than the intramedullary nailing group, and it has certain advantages in the treatment of long bone fractures of lower limbs, which is worthy of clinical application.
