| Objectives:To investigate the impact of 12-week regular exercise on the serum Irisin,OC,ADP levels and their relationship with glycolipid metabolism,islet β-cell function and insulin resistance status in patients with T2DM.Methods:60 patients with type 2 diabetes were randomly assigned to T2DM exercise group(n=30)and T2DM control group(n=30).20 matched healthy individuals were recruited in the normal control group.The T2DM exercise group performed regular exercise intervention for 12 weeks while the T2DM control group and the normal control group maintained the original lifestyle.Before and after exercise intervention,fasting plasm glucose(FPG),2-hour postprandial blood glucose(2hPBG),glycosylated hemoglobin(HbAle),fasting C-peptide(FCP),fasting insulin(FINS),total cholesterol(TC),triglyceride(TG),low density lipoprotein cholesterol(LDL-C),high density lipoprotein cholesterol(HDL-C)and serum Irisin,OC,ADP levels were measured.Anthropometric measurements were recorded for calculating body mass index(BMI),waist-to-hip ratio(WHR).The index of β-cell function(HOMA2-%B)and insulin resistance(HOMA2-IR)were calculated using HOMA2 calculator from the Oxford Center for Diabetes,Endocrinology and Metabolism.Then all data were analyzed with SPSS 23.0 statistical software.The normal distribution data were expressed as mean ±standard deviation(x±s).One-way ANOVA was used in the comparison between groups.Next,the homogeneity of variance with LSD-t method,the heterogeneity of variance with Talhane’s method were used for pairwise comparison.Two independent samples t-test were used for comparison between the two groups.Paired t-test was used before and after the intervention within two groups.The median(P25%,P75%)was used for the skewed distribution data.The Kruskal-Wallis test was used to compare the three groups while the Mann-Whitney test for comparison between the two groups.Wilcoxon Signed Ranks test was used before and after the intervention within two groups.The correlation analysis was performed by Spearman.When the significance test was set to P<0.05,the data difference was statistically significant.Results:1.Before exercise intervention,serum Irisin and ADP in T2DM exercise group and T2DM control group were lower than normal control group[16.98(14.55,26.86)pg/mL,19.07(15.62,23.79)pg/mL vs 37.44(23.16,50.15)pg/mL;100.00(86.63,139.63)μg/L,109.50(85.25,129.13)μg/L vs 180.00(126.50,250.75)μg/L,P<0.05],the BMI,HbA1c,FPG,TC,TG,LDL-C were higher but HDL-C was lower than normal control group(P<0.05).There was no significant difference between the two T2DM groups before exercise(P>0.05).2.After exercise intervention,the BMI,HbA1C,FPG,2hPBG,HOMA2-%B5 HOMA2-IR in the T2DM exercise group were significantly improved compared with the pre-exercise and T2DM control groups(P<0.05).Serum Irisin and ADP were significantly increased[33.53(18.04,42.28)pg/mL vs 16.98(14.55,26.86)pg/mL,17.46(15.15,23.23)pg/mL;153.00(101.88,197.38)μg/L vs 100.00(86.63,139.63)μg/L,107.50(87.00,155.00)μg/L,P<0.05].However,there was no significant difference in serum OC(P>0.05).3.Spearman correlation analysis showed that serum OC was negatively correlated with FPG,2hPBG and HOMA2-IR in T2DM patients(r=-0.273,-0.335,-0.281,P<0.05),positive correlation with serum ADP and Irisin(r=0.515,0.317,P<0.05).Serum Irisin was negatively correlated with 2hPBG(r=-0.264,P<0.05)and positively correlated with serum ADP(r=0.459,P<0.001).Before and after trial,the OC change rate was positively correlated with that of FCP change rate and HOMA2-IR change rate(r=0.278,0.304,P<0.05).The change rate of Irisin was negatively correlated with that of FPG and HOMA2-IR(r=-0.285,-0.350,P<0.05)and positively correlated with ADP change rate(r=0.329,P<0.05).Conclusions:Regular exercise resulted in improvement of serum Irisin and ADP levels,systemic metabolic markers,islet β-cell function and insulin resistance in patients with T2DM. |
