Clinical Study On The Mechanism Of The Necrosis-dispellingeffect Of Xinjia Chilin Powder On The Early Wound Of Anal Fistula

Objective:By comparing the effects of Xinjia Chilinsan dressing and sterilized vaseline gauze dressing on the early removal of decay of low simple anal fistula after operation,it is concluded that Xinjia Chilinsan has obvious effect on the promotion of removal of decay and tissue regeneration of postoperative wounds.The decrease of inflammatory factors and the increase of growth factors provide a reliable basis for clinical application.Method:From May 2019 to December 2019,60 patients with low simple anal fistula in Nanjing Hospital of traditional Chinese Medicine were randomly divided into two groups:the test group(30 cases of Xinjia Chilinsan dressing)and the control group(30 cases of sterilized vaseline gauze dressing).Six time points of day 2,day 7,day 10,day 14,day 21,day 28 were selected to observe and compare granulation growth,pain,healing time and other subjective indicators of wounds.The levels of TNF-α,IL-6 and VEGF were measured by means of ELISA.The overall clinical effect was evaluated in the 6th week after operation.Result:1.There was no significant difference in age,gender and course of disease between the two groups;there was no significant difference in wound surface between the two groups on the second day after operation.2.There was significant difference in wound secretion,granulation quality,time of decay removaland pain index between the two groups on the 7th,10th,14th and 28th day after operation,suggesting statistical significance(P<0.05).3.Comparison of wound healing time between the two groups:the overall healing time of the patients in test group was significantly shorter than that in control group,with an average of 3 days faster,and the difference was statistically significant(P<0.05).4.Analysis of laboratory indicators:on the 15th day after operation,a small amount of decay was taken for detection.The content of TNF-αand IL-6 in test group was lower than that in control group,and the content of VEGF in test group was higher than that in control group.5.On the overall efficacy,the wounds in both groups were completely healed,with a cure rate of 100%.Conclusion:Through the clinical experimental study of the effects of Xinjia Chilinsan dressingon the wound healing of low simple anal fistulaafter the operation,it is clear that it can effectively reduce secretion,promote the growth of granulation,accelerate the removal of decay,shrink the wound and shorten the healing time.It is preliminarily believed that the mechanism is:the content of TNF-α and IL-6 is reduced,and that of VEGF raised,which fully shows thatXinjia Chilinsan has anti-inflammatory and healing effects on the molecular biological level,and has clinical significance.
