Objective:Understanding cancer patients Implanted Venous Access Port(IVAP)chemoth-erapy self-care ability present situation; To explore the correlation between self-c are ability,self-management efficacy, social support and anxiety of tumor patients with IVAP chemotherapy. To analyze the factors influencing the self-care ability of tumor patients with IVAP chemotherapy. The purpose of this study is to pro vide scientific reference for the future development of nursing interventions to im prove the self-care ability of tumor patients with IVAP chemotherapy.Method:In this study,186 Patients admitted to a grade ? grade a tumor hospital in Liaoning Province and treated with IVAP for intravenous chemotherapy were selected from October 2018 to October 2019 by convenient sampling.General information questionnaire,Exercise of Self-care Agency(ESCA),Strategies Used by People to Promote Health(SUPPH),Social Support Rating Scale(SSRS)and Self-rating Anxiety Scale(SAS)were used.to conduct a uestionnaire survey on the research subjects. All data were input by Excel and analyzed by SPSS23.0 software.Frequency and composition ratio were used to represent the counting data, and mean ± standard deviation was used to represent the measurement data.Independent sample t test and/or one-way anova were used to compare the differences in self-care ability of tumor patients receiving IVAP chemotherapy among different basic data such as age and gender.Spearman correlation analysis was used to analyze the correlation betweenself-management efficacy, social support, anxiety and self-care ability of tumor patients with IVAP chemotherapy.Multiple stepwise regression was used to analyze the factors affecting the self-care ability of tumor patients with IVAP chemotherapy.P<0.05 was considered as statistically significant.Result:1 Current status of self-care ability of tumor patients with IVAP chemotherapyThe score of self-care ability of tumor patients receiving IVAP chemotherapy was(112.75±12.52).The self-care ability of 84 patients(46.7%)was at the high level, and that of 96 patients(53.3%)was at the medium level. The scores of each dimension were from the largest to the smallest: self-care responsibility(3.22±0.39),self-concept(2.88±0.37),health knowledge(2.46±0.34),and self-care skills(2.38±0.38).2 Univariate analysis of self-care ability of tumor patients with IVAP chemotherapyThe self-care ability of tumor patients receiving IVAP chemotherapy showed statistically significant differences in age, gender, educational level, marital status, family residence, occupation, family per capita monthly income, primary caregivers and medical payment methods(P<0.05).3 Correlation between self-care ability and self-management efficacy, social support and anxiety in tumor patients treated with IVAP chemotherapy was analyzedThe total self-management efficacy of IVAP chemotherapy in tumor patients was(103.57±11.81),of which 123(68.3%)patients were at medium level and 57(31.7%)patients were at high level. The scores of each dimension were from the largest to the smallest: positive attitude(3.96±0.38),self-decision-making(3.63±0.43),self-decompression(3.32±0.57).Efficacy of self-management,positive attitude,self-decision and self-decompression in three dimensions of IVAP chemotherapy in tumor patients were significantly positively correlated with self-care ability(r=0.535 ?0.948,P<0.01).The total score of social support for tumor patients receiving IVAP chemotherapy was(41.38±7.97),among which 113(62.8%)were at medium level and 67(37.2%)were at high level. The scores of each dimension from the largest to the smallest were:objective support(8.03±2.07),subjective support(2.55±0.37),and utilization of social support(2.37±0.45).The social support and its three dimensions of objective support, subjective support and utilization of social support in tumor patients with IVAP chemotherapy were significantly positively correlated with self-care ability(r=0.428 ?0.967,P<0.01).The total anxiety score of tumor Patients receiving IVAP chemotherapy was(50.19±6.41),and the incidence of anxiety was 56.1%.Among them,88 patients(48.9%)had mild anxiety and 13 Patients(7.2%)had moderate anxiety. The anxiety of tumor patients receiving IVAP chemotherapy was not correlated with their self-care ability(P>0.05),but was positively correlated with their health knowledge dimension(r=0.154,P<0.05).4 Multivariate analysis of self-care ability of tumor Patients with IVAP chemotherapyThe results of multiple stepwise regression analysis showed that social support,age,self-management efficacy, educational level and family residence were the influencing factors of Patients' self-care ability after IVAP chemotherapy, which accounted for95.6% of the variation of patients' self-care ability.Conclusion:1 The self-care ability of tumor patients with IVAP chemotherapy was generally at the middle and high level,in which the self-care responsibility score was the highest and the self-care skill score was the lowest.2 The self-management efficacy, social support and self-care ability of tumor Patients with IVAP chemotherapy were significantly Positively correlated, and the health knowledge of self-care ability and anxiety were significantly Positively correlated.3 Factors influencing of the self-care ability of tumor Patients with IVAP chemotherapy include social support,age,self-management efficacy, educational level and family residence. The younger the age,the higher the education level, the higher thesocial support level, and the higher the self-management efficacy level of tumor patients, the stronger the self-nursing ability of IVAP chemotherapy. The self-care ability of tumor patients living in urban or suburban areas was higher than that of those living in rural or township areas.
