Objective CHIVA is French for "conservative and hemodynamic treatment of outpatient venous insufficiency".It is an implicit treatment of chronic venous disease(CVD)of the lower extremities based on the concept of hemodynamics proposed by Claude Franceschi in 1988.The method has little damage to the patient.This study mainly discussed the clinical effect of hemodynamic correction on varicose veins of lower limbs,and provided certain reference for the clinical treatment of patients with deep veins of lower limbs.Methods Clinical data of 76 patients with varicose veins of lower limbs admitted to our hospital from December 2018 to June 2019 were collected for analysis.Patients were divided into CHIVA group and traditional surgical technique treatment(set as control group)according to the method of random grouping.To analyze and compare the operation time,hospital stay,intraoperative blood loss,hospital expenses,postoperative satisfaction,postoperative pain,postoperative complication rate and other aspects of different treatment schemes,and to compare the differences in the efficacy of patients with different CEAP stages.Results 1.The average operating time of patients in the CHIVA group was(59.13±10.34)min.The mean operation time of the control group was(61.33±8.41)min.There was no significant difference in data comparison(P > 0.05).The mean intraoperative blood loss in the CHIVA group was(13.41±3.67)ml.The mean intraoperative blood loss in the control group was(26.52±5.78))ml.There was a statistically significant difference in data comparison(P < 0.05).The mean hospital stay in CHIVA group was(4.25±1.26)ml.The mean hospital stay in the control group was(8.33±2.66)ml.There was a statistically significant difference(P < 0.05).The average hospitalization cost in CHIVA group was RMB(6873.54±301.22).The average hospitalization cost of the control group was(7633.67±611.45)yuan.There was a statistically significant difference in data comparison(P < 0.05).2.In the CHIVA group,there were 29 cases of mild postoperative pain,7cases of severe pain,and the incidence of severe pain was 19.4%.In the control group,there were 13 cases of mild postoperative pain and 27 cases of severe postoperative pain.The incidence of severe postoperative pain was 67.5%,with statistically significant difference(P < 0.05).3.The overall incidence of postoperative hematoma formation,skin ecchymosis,sensory disturbance,superficial phlebitis,saphenous nerve injury,wound infection and other complications in the CHIVA group was 11.1%;The total incidence of postoperative complications such as hematoma formation,skin ecchymosis,sensory disturbance,superficial phlebitis,saphenous nerve injury and wound infection in the control group was 35.0%.The total incidence of postoperative complications in the CHIVA group was significantly lower than that in the control group,with statistically significant differences(P < 0.05).4.The recurrence rate of patients in the CHIVA group was 2.8% 6 months after surgery;The recurrence rate of patients in the control group was 5.00% 6 months after surgery,and that of patients in the CHIVA group was significantly lower than that of the control group,with statistically no significant difference(P >0.05).Postoperative satisfaction score of CHIVA group was(4.42 ± 0.48).The postoperative satisfaction score of the control group was(2.24 ± 0.61),and that of the CHIVA group was significantly higher than that of the control group(P < 0.05).5.The mean preoperative blood flow velocity of deep vein in CHIVA group was(5.49±1.18)cm/s.The mean preoperative blood flow velocity of the deep vein in the control group was(5.44±1.19)cm/s.There was no significant difference in data comparison(P > 0.05).The mean deep vein flow velocity of CHIVA group was(2.09±0.65)cm/s 3 months after surgery.The mean preoperative blood flow velocity of the deep vein in the control group was(3.22±0.83)cm/s.The data of CHIVA group was significantly lower than that of control group,and there were significant statistical differences in data comparison(P < 0.05).The mean preoperative superficial vein flow velocity of CHIVA group was(3.72±1.01)cm/s.The mean preoperative blood flow velocity of the deep vein in the control group was(3.69±1.04)cm/s.There was no significant difference in data comparison(P > 0.05).The mean flow velocity of superficial vein in CHIVA group was(2.13±0.61)cm/s 3 months after surgery.The mean preoperative blood flow velocity of deep vein in the control group was(2.63±0.59)cm/s.The data of CHIVA group was significantly lower than that of control group,with statistically significant difference(P < 0.05).6.CEAP grading method was used to evaluate the total clinical response rate of patients in c2-c4 group,and the total clinical response rate of patients in CHIVA group was 96.43%,which was higher than that of patients in control group(90.00%).CEAP grading method was used to evaluate the total clinical response rate of patients in the c5-c6 group,and the total clinical response rate of patients in the CHIVA group was 75.0%,which was higher than that in the control group(60.0%).There was no significant statistical difference(P > 0.05).Conclusion For varicose veins of the lower extremities,CHIVA therapy is effective and safe,and will not cause major trauma,and can recover as soon as possible,reduce the cost of treatment.The incidence of complications and recurrence rate are significantly reduced,and it also shows significant advantages in terms of patient satisfaction.
