| Objective:An animal model of peripheral nerve injury and repair was established,and then high concentration of platelle-rich plasma and PRP thrombin were injected into the injured peripheral nerve to explore the effect of PRP on peripheral nerve injury and further explore its possible mechanism.Methods:1.A total of 30 healthy white rabbits were randomly divided into three groups:group 1(normal saline group),group 2(PRP group),and group 3(PRP thrombin group).Both male and female,8-10 weeks of age,weight of 1.0-1.5kg.2.2.Establishment of animal models for suture repair of fractured sciatic nerve:in group 2 and group 3,high concentration PRP was prepared by landes-berg centrifugation method twice after cardiac blood collection with an anticoagulant syringe.After successful abdominal anesthesia,the skin was prepared in the surgical area,and disposable sterile tissue was applied after disinfection.The incision was made longitudinally,about 6cm in length,from the muscle gap between the posterior lateral thigh biceps femoris and tensor fascia lata.While searching for the sciatic nerve to separate the muscle gap without cutting the muscle to ensure the muscle integrity,the surgical field exposed about 4cm sciatic nerve,The sciatic nerve was cut vertically and quickly by microsurgical cutting.Under the microscope,the outer membrane of the broken end of the nerve was uniformly sutured with 7-0 noninvasive nerve suture,and the nerve bundle branches were not exposed or excessively pulled.Group 1 was injected with 0.2ml normal saline,group 2 with 0.2ml PRP,and group 3 with 0.2ml PRP thrombin.The incision was then split with a 7-0 suture.After waking up,feeding in strict groups and cages,penicillin 200000 U,intramuscular injection,qd × 3d,Prevent infection,limit strenuous activity,and keep in the same laboratory.3.Observe indicators:3.1 At the 8th week after the operation,30 white rabbits were sacrificed by the same method,and the triceps muscle of the lower leg was completely dissected and the wet weight of the triceps muscle of the lower leg was measured.3.2 The triceps muscle of the calf was cut vertically with a scalpel,and the muscle was fixed with a continuous semi-thin section for specimen.Section was made and the number of triceps muscle fibers was observed under light microscope.3.3 The 2cm nerve from the center to the distal end and the proximal end of the sciatic nerve was taken as the specimens,and the electrophysiological monitoring was carried out to observe the action potential.3.4 The nerve specimens were fixed by continuous semi-thin sections,and then sections were made.The number of nerves was counted by light microscope.3.5 SPSS 21.0 statistical software package carries out statistical analysis on the data.Results:In group 3,the number of action potential and nerve repair of sciatic nerve at 8 weeks after surgery was higher than that of group 1 and group 2,and the number of muscle fibers of the affected limb in group 3 was higher than that in group 1 and group2.The muscular atrophy of the affected limb in group 3 was better than that in group 1 and group 2,these all differred significantly.Conclusion:Percutaneous injection of PRP thrombase after suture of sciatic nerve fracture in rabbits can promote the repair of nerve action potential,nerve number and muscle fiber of the affected limb,which is beneficial to the regeneration and restoration of peripheral nerves. |
