Objective: Retrospective studies analyzed the effects of infections on the prognosis of cirrhosis with liver failure. It provides a more precise guide for prevention and treatment, thereby reducing the mortality of patients with liver failure. Methods: I collected 68 patients with cirrhosis and liver failure, which admitted to our hospital from June 2016 to June 2019 and divided them into 50 cases of infection group and 18 cases of non-infection group;and divided 50 patients that infection in cirrhosis with liver failure into survival group and death group;collected the length of hospital stay, gender, age, number of infected sites, etiology of liver failure, type of liver failure, laboratory tests,MELD score, complications, etc.analyzed the clinical and laboratory indicators for differences between the survival and death groups. And I took the discharge or death at the hospital as the cut-off point, and statistical method using two-class logistic regression to analyze the risk factors and independent influencing factors of infection affecting the prognosis of cirrhosis with liver failure.Results:1.A total of 68 patients were enrolled in this study,of which 50 were infected and 18 were uninfected, and the infection rate was 73.5%(50/68).2.68 patients with cirrhosis and liver failure had 22 deaths with a mortality rate of 32.4%.68 patients were divided into infection group and non-infection group,a total of 50 patients in the infection group, including 20 deaths and a mortality rate of 40%(20/50),18 patients in the non-infection group, including 2 deaths and a mortality rate of 11.1%(2/18),and there was a statistical difference between the two groups(?2=5.047,P=0.025).3.I compared the survival and death groups between groups, presence of infection, length of hospital stay, number of infected sites, type of liver failure, white blood cell count, neutrophil ratio, neutrophil count,neutrophil-lymphocyte ratio(NLR),total bilirubin(TBIL),hyponatremia, urea nitrogen, creatinine, prothrombin activity(PTA),international standard ratio(INR),MELD score, hepatorenal syndrome. There are statistical differences.4.The length of hospital stay, the number of infected sites, type of liver failure, neutrophil count,NLR,TBIL,creatinine, hyponatremia,PTA,INR,MELD score, and hepatorenal syndrome are statistically significant risk factors affecting the prognosis of cirrhosis with liver failure.5.After co-linear diagnosis and excluded other influencing factors,The length of hospital stay(P=0.027),neutrophil count(P=0.034),MELD score(P=0.026),and hepatorenal syndrome(P=0.050)are a statistically significant independent risk factor for prognosis in patients with cirrhosis and liver failure and infection. The length of hospital stay, neutrophil count, and MELD score of the death group were significantly higher than those of the survival group.Patients with hepatorenal syndrome have the highest mortality rates. Conclusion: 1.The mortality of patients with cirrhosis and liver failure and infection was significantly increased.2.The length of hospital stay, the number of infection sites, the type of liver failure, neutrophil count,NLR,TBIL,creatinine, hyponatremia,PTA,INR,MELD score, and hepatorenal syndrome are risk factors that affect the prognosis of infection of cirrhosis with liver failure.3.The length of hospital stay, neutrophil count,MELD score, and hepatorenal syndrome were independent risk factors affecting the prognosis of infection with cirrhosis and liver failure.
