Objective: The levels of carcinoembryonic antigen(CEA)and interleukin-17(IL-17)in serum and cerebrospinal fluid of patients with lung cancer were detected to explore the diagnostic value of CEA and IL-17 levels in brain metastasis of lung cancer.Methods:A total of 82 lung cancer patients who were hospitalized in the Oncology Department of Shaoyang Center Hospital from July 2018 to December 2019 and suspected of brain metastasis were selected as the research objects.According to the results of head CT and/or MRI examination,58 patients with brain metastasis of lung cancer and 24 patients with non-brain metastasis of lung cancer were judged.The levels of CEA in serum and cerebrospinal fluid of each patient was detected by direct chemiluminescence.Cytometric Bead Array(CBA)was employed to detect the levels of IL-17 in serum and cerebrospinal fluid of each patient.Receiver operating characteristic curve(ROC)analysis was used to find the cut off of CEA and IL-17 in the diagnosis of brain metastases from lung cancer and the diagnostic value of combineduse of CEA and IL-17 for brain metastases from lung cancer.Results:(1)The levels of CEA and IL-17 in serum and cerebrospinal fluid of patients with brain metastases from lung cancer are higher than those of patients with non-brain metastases.The comparison between the two groups is statistically significant(P<0.05);The levels of CEA and IL-17 in cerebrospinal fluid of lung cancer patients with non-brain metastasis are lower than those in serum(P<0.05),while the levels of CEA and IL-17 in cerebrospinal fluid of lung cancer patients with brain metastasis are higher than those in serum(P<0.05).(2)Diagnostic significance of serum CEA and IL-17 levels for brain metastasis of lung cancer: Through ROC analysis,the cut off of serum CEA is 11.92,the sensitivity is 0.534,the specificity is 1.000,the Youden ? s index is 0.534,and the AUC is 0.685(95%CI :0.574,0.797).The cut off of serum IL-17 is 24.16,the sensitivity is 0.362,the specificity is 1.000,the Youden ? s index is 0.362,and the AUC is0.644(95%CI:0.525,0.762).(3)Diagnostic significance of cerebrospinal fluid CEA and IL-17 levels for brain metastasis of lung cancer: Through ROC analysis,the cut off of cerebrospinal fluid CEA is 9.365,the sensitivity is 0.724,the specificity is 1.000,the Youden?s index is 0.724,and the AUC is 0.848(95%CI:0.768,0.929).The cut off of cerebrospinal fluid IL-17 is 20.31,the sensitivity is 0.741,the specificity is 0.958,the Youden?s index is0.700,and the AUC is 0.843(95%CI:0.758,0.928).(4)Diagnostic significance of combined application of serum CEA and IL-17 levels for brain metastasis of lung cancer: The sensitivity is 0.207,the specificity is 0.958,and the Youden ? s index is 0.207 in serial test of serum CEA and IL-17.The sensitivity is 0.690,the specificity is 1.000,and the Youden?s index is 0.690 in parallel test of serum CEA and IL-17.(5)Diagnostic significance of combined application of cerebrospinal fluid CEA and IL-17 leves for brain metastasis of lung cancer: The sensitivity is 0.500,the specificity is 1.000,and the Youden?s index is 0.500 in serial test of cerebrospinal fluid CEA and IL-17.The sensitivity is 0.965,the specificity is 0.958,and the Youden?s index is0.923 in parallel test of cerebrospinal fluid CEA and IL-17.Conclusions:(1)Levels of CEA and IL-17 were elevated in serum and cerebrospinal fluid of patients with lung cancer,which is helpful for the diagnosis of brain metastases from lung cancer.(2)Combined application of CEA and IL-17 levels in serum or cerebrospinal fluid of lung cancer patients can provide reference for early diagnosis of brain metastases from lung cancer.
